The project that seeks to cancel more than 30 Official Mexican Standards (NOM) for health has generated a strong discussion. Despite the arguments presented by the federal authorities, it has been questioned whether it really is a decision that offers benefits to the population. It is even mentioned that it is a setback that also affects medical personnel in the performance of their duties.

Everything was generated by the initiative of the Dr. Hugo López-Gatell, who heads the Standardization Committee of the Ministry of Health (SSa). In his case, on June 1, he published in the Official Gazette of the Federation (DOF) the supplement to the National Quality Infrastructure Program 2023, for all standardization authorities of the Federal Public Administration (APF).

As part of this initiative, various health NOMs could disappear. Within the list there are some related to breast cancer, breastfeeding and leptospirosis, among others.

What will happen if the proposal is approved?

The Network for Comprehensive Care of Chronic Diseases (RAIEC)together with more than 100 signatory civil organizations, expressed their deep concern about this proposal.

The first thing he mentions is that within the list there are seven related to chronic noncommunicable diseases (NCDs). They add that a decision of this nature should be widely justified and agreed, since it could violate and put people at risk.

“80% of deaths in Mexico are caused by some CNCD, so the cancellation of these NOMs is unacceptable, since it will lead to precarious care for people and without minimum parameters to be met by health service providers. public and private health.

In this vein, conditions such as overweight and obesity; dyslipidemias; Mellitus diabetes; systemic arterial hypertension; benign prostatic growth and prostate cancer; cervical cancer and breast cancer are among the main public health problems in Mexico.

Health is a human right and, as such, the principle of progressivity must be respected. Before canceling the NOM for CNCDs, new regulations should be updated and created for care of other types that do not yet have a quality standard.

Organizations demand explanations from federal authorities

The RAIEC requests different authorities to act for the benefit of people living with chronic conditions. It is necessary that the competent authorities, in accordance with their powers and areas of responsibility:

  • Suspend the NOM cancellation process until the consequences for patient care are identified.
  • Guarantee citizen participation and, in conjunction with patient organizations and medical experts, evaluate the relevance of updating the NOMs, instead of concluding the cancellation process.
  • Inform what the references will be for the services provided in the public and private sectors, so that both medical professionals and users of the health systems are guaranteed:

○ Legal certainty.
○ Professional responsibility.
○ Parameters for decision making and approaches to care based on scientific evidence.
○ Patient safety.

Canceling seven NOMs for the care and treatment of CNCDs could cause further deterioration of the Mexican health system, loss of life in the near future, and the development of comorbidities.

The precariousness of the service will have consequences in the increase in people’s out-of-pocket spending and greater pressure on public finances due to health complications linked to the lack of minimum standards of care.

The authorities must prioritize citizen participation, as well as consultation with experts in all regulatory processes that have possible consequences for the health of Mexicans. Adding expert voices and placing the patient at the center of decisions strengthens institutions.
