STI Diagnoses Surge in 2022: Gonorrhea, Chlamydia, Syphilis, and More

Of all STIs, the increase in gonorrhea was greatest in 2022. With 10,600 diagnoses, this is an increase of 33 percent compared to 2021, when there were 7,964 cases. The number of women and heterosexual men with gonorrhea has increased sharply. Among women, the percentage of tests that were positive increased from 1.5 percent in 2021 to 2.3 percent in 2022, and among men that percentage rose from 1.8 percent to 2.4 percent. For women, this means the highest percentage since 2013. The increase was mainly seen in the second half of 2022. Among men who have sex with men (MSM (men who have sex with men)) who do not participate in the PrEP pilot, the percentage increased slightly, from 12.4 to 12.8 percent. Among MSM who do participate in the PrEP pilot, the percentage went from 9.2 to 9.8 percent.

Chlamydia

In 2022 there were 24,684 chlamydia diagnoses, an increase of 21 percent compared to 2021. The percentage of women with chlamydia was higher in 2022 than in 2021: it rose from 16.2 to 17.9 percent. The percentages of heterosexual men and MSM with chlamydia fell slightly to 21.2 and 10.9 percent respectively.

Syphilis

The number of syphilis diagnoses was also higher at 1,574 than in 2021, when there were 1,398. But a slight decrease in the percentage of positive tests was visible among MSM: from 2.6 percent in 2021 to 2.3 percent in 2022. Among MSM participating in the PrEP pilot, the percentage remained the same (1.7 percent). The number of diagnoses among women and heterosexual men remained low in 2022: 0.29 percent among heterosexual and 0.15 percent among women.

Hiv

In 2022, 144 people received an HIV diagnosis at the CSGs. That is slightly more than in 2021, when it was 138 people. Of those 144 persons, 10 were diagnosed in participants in the PrEP pilot, of which four persons were diagnosed upon entry into the pilot. The number of diagnoses in women was eleven and eight in heterosexual men. In 2022, 997 people with HIV went to an HIV treatment center (‘in care’) for the first time. That is 26 percent more than in 2021 (794 people).

PrEP-pilot

Since August 2019, the CSGs also provide care to people who use PrEP. PrEP is a drug that prevents HIV. Men who have sex with men (MSM) and other people with an increased risk of HIV can participate in a pilot in which they receive PrEP. These participants in the PrEP pilot are tested for STDs every three months.

At the end of 2022, the PrEP pilot program had an estimated 8,558 participants. The percentage of participants with an STI was 39.8 percent in 2022.

Mpox

In May 2022, the first cases of mpox (previously known as monkeypox) were reported in the Netherlands. A total of 1,259 mpox infections were reported to RIVM last year, 92 percent of which were among MSM.
