TITRE : RECRUITMENT OF A NATIONAL CONSULTANT FOR CARRYING OUT THE NATIONAL SENTINEL SURVEILLANCE SURVEY FOR HIV AND SYPHILIS AMONG PREGNANT WOMEN RECEIVED IN CPN (ENSS), GUINEA, 2023

The Republic of Guinea requested and obtained a grant from the Global Fund to Fight AIDS, Tuberculosis and Malaria for the period 2021-2023. Part of this grant is intended to reduce mortality and morbidity among adults and children living with HIV through improved access to treatment (including treatment of TB/HIV co-infection), care and support. people living with HIV.

The HIV/AIDS epidemic in Guinea, as in other countries in the West African sub-region, is generalized with a national prevalence of 1.5% among the population aged 15 to 49 (EDS 2018). It varies according to the area of ​​residence (2% in urban areas compared to 1.1% in rural areas) and administrative regions (highest in Boké 2% followed by Kindia with 1.8% and Conakry with 1.7%).

According to Spectrum v6.29 database Guinee_2023_v2 projections, the number of people living with HIV in Guinea in 2022 was 117 763 [102 140– 136 998] dont 75,327 women [65 049– 87 742] or 64%. We observe a feminization of HIV infection (whether it concerns the prevalence of HIV observed from surveys[1] or that observed from estimates[2]).

Regarding pregnant women, although the method of recording prenatal consultation data is strongly recommended in terms of surveillance. The situation in the Republic of Guinea is such that the databases available at the operational level are not yet sufficiently consistent and stable to effectively meet the need for prevalence data among pregnant women. Indeed, on the one hand, the DHIS-2 retained as a routine data communication system, continues its consolidation phase with a view to achieving a proven degree of accuracy, completeness and integrity of the data. Other screening at several PMTCT sites in the country, which does not guarantee a continuous offer of screening for pregnant women seen in CPN.

Until suitable conditions are continually met, surveillance of HIV and syphilis among pregnant women receiving ANC in the Republic of Guinea will essentially rely on sentinel surveillance by convenience sampling of prenatal consultation sites representing all regions of the country. The monitoring and evaluation plan of the National Program provides for annual completion of the sentinel surveillance survey.

Thus, according to the latest survey, carried out in 2020, the median HIV seroprevalence was 1.5% [1,27-1,73] and that of syphilis was 0.7%. This seroprevalence varied depending on the region. For HIV: 3.4% (Boké), 1.9% (Conakry), 1.5% (Kankan), 1.4% (N’zérékoré and Kindia) and the lowest were observed in the regions of Faranah 0.95%, in Mamou and Labé 1.0%. For syphilis: 2.8% (Boké), 1.6% (Faranah) and 1.3% (Kindia).

According to the DHIS2, Guinea has a total of 521 sites offering the Prenatal Consultation (ANC) and catch-up screening service which reported data during the year 2022. During previous HIV Sentinel Sero-surveillance surveys in 2004 in 2020, a very small number of sites (17 – 43) participated in these surveys.

Although the number of sentinel sites has evolved, the level of representativeness still needs to be improved. Hence the non-representativeness of the prevalence data at the national level.

Thus, to compensate for this weakness, the survey proposes for this year 2023, to select three (3) health structures for certain health districts with high attendance and two (2) sites for other health districts (38 health districts ) i.e. a total of 76 health structures including the 43 sites which participated in the 2020 study, in order to reduce the limits observed in terms of representativeness of the sites of previous surveys.

To conduct this survey for the year 2023, this notice of expression of interest is launched to recruit a national consultant who will support the PNLSH in carrying out the survey..

Participation in the competition is open on equal terms to all individual consultants with sufficient qualification and expertise required by type of consultancy.

Interested candidates are invited to express their interest in the provision of the services described in the Terms of Reference, a copy of which is obtained upon request sent to the following email address: [email protected] with details of the consultancy(s). (s).

Applicants are requested to provide information indicating that they are qualified to perform the services described, in their Expression of Interest files prepared and presented separately as follows:

Technical proposition : including the cover letter/explanatory note, the detailed methodological approach, the detailed work plan and timeline, the well-detailed and updated CV best describing the consultant’s expertise with copies of diplomas and professional certificates, as well as references similar with certificates of service rendered, satisfaction and/or recommendations.Financial proposal : including all costs relating to the mission including fees and possibly other necessary reimbursable costs. Please note that the financial proposal must be made excluding VAT because the project is exempt from VAT and the Consultant will be subject to the taxation applicable in the Republic of Guinea.

The candidate will be selected based on the qualification of the consultant (see paragraph 7 of the ToR).

Proposals drawn up in French in two copies, one (1) original and one (1) copy must be submitted from the UAGCP Procurement Department located on the 3rd floor of the Palm Résidence Camayenne building, north cornice of Dixinn, next to the Ambroise Paré clinicat the latest Tuesday 14/11/2023 at 11:00 a.m. sharp, local time.
