Causes and Treatments of ‘Double Vision’
Monocular diplopia is caused by cataracts, etc.
Binocular diplopia is a disorder of the ocular muscle nerve system.
If you have eye pain, etc., get checked out quickly.

If you suddenly see two objects, you may be confused about whether to go to an ophthalmologist or a neurologist. In fact, the eyes and the brain are very closely related. The eyes, which receive visual information from the outside, are connected to the brain through the optic nerve. Decreased vision and double vision, which is when objects appear double, are typical symptoms of vision abnormalities. Depending on the suspected disease, the department you should visit may vary.

Double vision is a phenomenon in which one object appears as two or a shadow appears. The cause of double vision varies depending on whether the symptom occurs in only one eye or both eyes, so you should close your eyes one by one to see which side has the double vision.

Diplopia is divided into monocular diplopia and binocular diplopia. If diplopia occurs when you close one eye at a time when you have diplopia symptoms, you may suspect monocular diplopia. The most common causes of monocular diplopia include refractive errors in the eye and weakening of the muscles responsible for eye movement. In the case of monocular diplopia in the elderly, it is often caused by cataracts. In addition, dry eye, corneal wounds, and retinal diseases may be the causes, so it is recommended to visit an ophthalmologist.

Kim Joo-yeon, director of the ophthalmology department at Seran Hospital, said, “Monocular diplopia occurs when the transmission of light through the eye to the retina is distorted.” She added, “The most common causes are opacity of the lens, refractive errors, and in very rare cases, a tumor at the back of the eye can be the cause.” She continued, “People with diplopia in one eye usually visit an ophthalmologist to check their vision, pupillary response to light, and eye movement.”

Director Kim said, “Double vision is easy to diagnose in adults because they can describe what they see well, but it is difficult to diagnose double vision in children.” He emphasized, “If a child rubs his eyes a lot, looks sideways, or tilts his head to one side, he should be suspected of having double vision.”

If you see two things only when both eyes are open and it is fine when one eye is closed, you may suspect binocular diplopia. In particular, you should pay close attention to whether binocular diplopia symptoms persist or appear and disappear. Binocular diplopia can mainly be caused by problems with the alignment of the two eyes or by problems with the nervous system of the muscles that move the eyeball.

If double vision persists, it may be due to a nervous system disorder. Stroke may also be suspected, and double vision caused by a nervous system disorder may be accompanied by strabismus, limiting eye movement. Depending on the nervous system disorder, symptoms other than double vision may appear, such as speech or language problems and sensory loss. In particular, double vision that appears in the elderly may be suspected to be related to underlying diseases such as diabetes, arteriosclerosis, hypertension, and cerebral aneurysm.

Eun-ju Lee, head of the neurology department at Seran Hospital, emphasized, “Diplopia in both eyes can be caused by inflammation or damage to the extraocular muscles, or by problems with the cranial nerves, so various treatments such as ophthalmology, neurology, and internal medicine may be required depending on the doctor’s judgment.” She continued, “In particular, diplopia caused by nervous system abnormalities can be accompanied by strabismus, which limits eye movement. In this case, you should visit the hospital immediately and undergo a detailed examination such as an MRI.”

The manager explained, “Double vision can be caused by microscopic damage to the peripheral nerves, but it can also be a symptom of brain diseases such as cerebral infarction or brain tumors.” He continued, “Double vision is a symptom that requires an accurate diagnosis, so if you feel pain in your eyes or head, feel paralysis in your face, or have trouble maintaining your balance, you should quickly visit a hospital.”
