Rising Threat: Inflammatory Enteritis from E. coli surges in Spain,Driven by aging Population

A nationwide analysis reveals a significant increase in hospitalizations and deaths linked to inflammatory enteritis caused by diarrheagenic Escherichia coli (DEC) in Spain,with the elderly disproportionately affected. the study, published in Preventive Medicine Reports, identified a staggering 265,754 hospitalizations over a seven-year period, highlighting a growing public health concern.

A Growing Global Challenge

Diarrheal diseases remain a leading cause of mortality worldwide, particularly among children under five and the elderly. The World Health Association (WHO) identifies these illnesses as the second leading cause of death in young children globally. Among the many pathogens responsible for infectious diarrhea,diarrheagenic E. coli (DEC) stands out as a major contributor to both isolated cases and widespread outbreaks.

DEC encompasses several categories, all of which – except enterotoxigenic strains (ETEC) – can trigger inflammatory enteritis. This condition, characterized by intestinal inflammation, manifests in a spectrum of severity, ranging from mild mucosal involvement to severe ulcers, and presents with symptoms like abdominal pain, dysentery, watery diarrhea, fever, and the presence of blood or mucus in stool. Inflammatory enteritis caused by DEC presents unique challenges regarding both mortality and illness, especially for individuals with weakened immune systems or pre-existing health conditions.

Hospitalization Rates on the Rise

The recent study confirms the significance of this disease and reveals a concerning trend: hospitalization rates have increased substantially between 2016 and 2022, climbing from 81.9 to 94.7 per 100,000 inhabitants. This increase underscores the urgent need for proactive public health measures.

The vulnerability of Aging and Infants

The burden of this disease falls most heavily on the very young and the very old.Individuals over 80 consistently exhibit the highest hospitalization rates, reaching 516.7 per 100,000, peaking at an alarming 636 per 100,000 in 2022. This is attributed to the natural decline in immune function and the increased prevalence of comorbidities associated with aging.

Conversely, infants under one year of age experience the highest hospitalization rates within the pediatric population, at 227.1 per 100,000. While this rate has decreased slightly during the study period to 198.1, it remains significantly higher than rates observed in other pediatric age groups.

Interestingly, while women accounted for the majority of overall hospitalizations (53.9%), men exhibited higher hospitalization rates than women across most age groups, particularly among infants and older adults.

Mortality Rates Escalate

The impact of diarrheagenic E. coli enteritis extends to mortality, with 24,455 deaths among hospitalized patients in Spain between 2016 and 2022. The number of deaths rose significantly, from 2,643 in 2016 to 4,676 in 2022.

Consistent with hospitalization trends, those over 80 years of age were most affected by mortality, accounting for 53.8% of all deaths registered (13,163 deaths). While women experienced a higher total number of deaths than men (13,061 versus 11,394) and higher mortality rates in most age groups,a multivariable analysis revealed no significant difference in mortality risk between genders (Odds Ratio for men: 1.01).

Identifying Key Risk Factors

Researchers employed advanced statistical modeling to pinpoint factors that elevate the risk of death from diarrheagenic E.coli infection.Age emerged as the strongest predictor of mortality,with individuals 80 years or older having an 8.59 times higher probability of dying compared to infants.

Beyond age, the presence of specific comorbidities and co-infections dramatically worsened the prognosis. Sepsis was identified as the strongest comorbidity predictor, increasing the risk of mortality by a factor of 3.60. Cancer followed as the second most significant comorbidity, raising the risk of death with an Odds Ratio of 2.23.
