Americas Loses Measles-Free Status as Outbreaks Surge in Mexico and US
The Pan American region,once a global leader in public health,has relinquished its measles-free status due to sustained transmission of the disease,a setback attributed to lagging vaccination rates. The announcement, made this week by the Pan American Health organization (PAHO), underscores a growing vulnerability across the Americas despite ongoing efforts to combat the highly contagious virus.
A Reversible Setback
“This represents a setback, but it is a reversible situation,” stated jarbas Barbosa, director of PAHO, following a meeting of experts in Mexico City.The Americas had previously achieved measles-free status twice, most recently in 2024, making it the only region in the world to do so. however, Canada’s failure to interrupt transmission for 12 consecutive months triggered the loss of certification.
Despite the regional decline in measles elimination, several countries are grappling with significant outbreaks. Mexico and the United States are experiencing a resurgence of the disease, with a concerning emergence on both sides of the border linked to communities with low immunization rates. To achieve population immunity, coverage exceeding 95% with two doses of the measles vaccine is crucial, yet the region currently stands at 79%, with mexico lagging further behind at 69%.
Mexico Faces Mounting Crisis
Mexico is currently battling a national measles outbreak that began in March and has now persisted for eight months.To date, the outbreak has resulted in 23 deaths – 21 in the northern border state of Chihuahua – and 5,185 confirmed cases across 27 of the country’s 32 states.health officials estimate that the number of possible, unconfirmed infections is more than double the confirmed count.
while Mexico retains its individual measles-free certificate for now, the threat of losing it looms large. PAHO has acknowledged the country’s efforts to contain the outbreak but cautioned against delaying further vaccination campaigns. The currently scheduled follow-up campaign is not until April 2026,a timeframe officials deem too distant given the escalating situation.
Vulnerable Populations and Geographic Challenges
The outbreak in chihuahua is particularly challenging, fueled by a combination of factors. The sheer scale of the outbreak, coupled with vaccine hesitancy within some Mennonite communities, and the rugged terrain hindering access to remote populations, are all contributing to the rapid spread.
Infections are spreading among seasonal agricultural workers who travel between mountainous regions and Mennonite farms, effectively carrying the virus to previously unaffected areas.In response, the Mexican government extended the recommended vaccination age from under 40 to under 49 in July, aiming to protect this high-risk group.
PAHO Support and Urgent Action needed
PAHO is dispatching a support mission to Chihuahua this week to assist with professional training, laboratory surveillance, and vaccine access – identified as one of the most significant hurdles. According to Barbosa, ensuring vaccine availability at health centers, even on weekends, is paramount to overcoming barriers to immunization. Equally important is clear communication from medical personnel, translating the benefits of vaccination into accessible language for communities harboring doubts fueled by misinformation.
The organization emphasized the proven effectiveness of measles immunization,noting that it prevented an estimated 60 million deaths worldwide between 2000 and 2023,including 6 million in the Americas.
