Nemours Children’s Health Pioneers ‘Advanced Care at Home’ Program, Reducing Hospital Visits and Readmissions
A new initiative is bringing thorough paediatric care directly into patients’ homes, leveraging technology and 24/7 clinical support to improve outcomes and family well-being.
US-based Nemours Children’s Health has launched advanced Care at Home, a groundbreaking paediatric care program designed to deliver support to eligible children in their homes with continuous virtual access and clinical oversight. The program, which began enrolling patients in June, represents a meaningful shift towards patient-centered care and aims to redefine the landscape of paediatric healthcare.
The initiative specifically targets stable patients who still require enhanced medical care, with a particular focus on those with complex conditions and other medical diagnoses.A key objective is to accelerate safe discharge from hospitals and ensure continuity of care, minimizing disruptions to a child’s life and maximizing family comfort.
“Advanced Care at home is an extension of our Whole child Health approach – care designed to support children where they live, learn, and grow,” stated R Lawrence Moss, president and CEO of Nemours Children’s Health. “This is an entirely new model of care that brings children home, keeps families together, and lets the entire family be pleasant in their own space.”
Early results demonstrate the program’s effectiveness. To date, more than 120 patients have been enrolled, and Nemours reports a substantial positive impact on healthcare utilization.According to a company release, the program has successfully avoided 91 emergency department visits, returned 177 hospital days to families, and prevented 27 readmissions of inpatients.
The success of Advanced Care at Home hinges on a combination of technology-enabled monitoring and round-the-clock clinical access. This allows healthcare professionals to remotely monitor patients’ conditions, intervene proactively, and provide timely support, reducing the need for in-person visits and hospitalizations.
Jane Mericle, executive vice-president, chief nursing executive and patient operations officer at Nemours Children’s Health, emphasized the benefits for both patients and families. “We know patients and families benefit significantly from treatment options that enable safe, continuous care and recovery in the comfort of their own homes,” she saeid. “Advanced Care at Home offers tremendous promise in revolutionising paediatric healthcare.”
Looking ahead, Nemours Children’s Health plans to expand the program to Nemours Children’s hospital, Delaware by the end of 2025. Furthermore, a paediatric-focused mobile integrated health program is anticipated to launch in 2026, further extending the reach of innovative care solutions.
