Semaglutide Weight Loss Shows No Immediate Bone Density Harm in Older Adults, Study Finds

A small but carefully monitored 20-week study reveals that semaglutide delivers weight loss in older adults without immediate harm to bone density, raising new questions about what longer-term use of GLP-1 drugs may mean for aging bones.

Obesity is a growing concern for older adults in the United States, affecting around 40% of this population. While weight loss can improve chronic health conditions, concerns have lingered regarding its potential impact on bone health. New research published in Frontiers in Aging offers a promising, though preliminary, look at the effects of the popular weight loss drug semaglutide on bone density in older individuals.

Semaglutide and the Promise of Weight Management

Researchers investigated the effects of semaglutide, a glucagon-like peptide 1 receptor agonist (GLP-1RA), on bone health in older adults. Participants in the study had a body mass index (BMI) between 27 and 34.9 kg/m2 and dL, or glycated hemoglobin levels between 5.7% and 7.5%. Individuals with type 1 diabetes, cardiovascular issues, impaired kidney function, or uncontrolled hypertension were excluded from the trial.

Participants were randomly assigned to either a lifestyle counseling program combined with semaglutide therapy or lifestyle counseling alone. The lifestyle counseling involved guidance on exercise, diet, and behavioral changes based on the Diabetes prevention Program recommendations. The semaglutide group received weekly injections, starting at 0.25 mg/week and increasing to 1 mg/week over 12 weeks, alongside encouragement for moderate-intensity physical activity and resistance training. Researchers used dual-energy x-ray absorptiometry scans to assess body composition and bone mineral density (BMD) in the leg, lumbar spine, and pelvis at the begining and end of the 20-week period. Blood samples were also collected to measure bone turnover markers, specifically collagen type 1 C-telopeptide (CTX) and procollagen type 1 intact N-terminal propeptide (P1NP).

Weight Loss Achieved, But Bone Density Remains Stable

The results indicated that the group receiving semaglutide alongside lifestyle counseling experienced significantly greater weight loss – an average of 4.9 kg – compared to the lifestyle counseling-only group, which lost 0.8 kg.However, there were no statistically significant differences in whole-body composition or regional BMD values between the two groups. Despite this, researchers noted a consistent, though not statistically significant, trend toward lower BMD in the semaglutide group at the end of the study.

Further analysis revealed no significant differences in bone turnover markers (P1NP and CTX) between the groups.Changes in body weight were weakly correlated with changes in BMD in most areas, but a significant and strong association was observed between weight loss and changes in pelvis BMD. Body weight changes were also inversely, but weakly, correlated with changes in P1NP and CTX.

Cautious Optimism and the Need for Further Research

The study’s findings suggest that, over a 20-week period, semaglutide-induced weight loss does not appear to cause measurable harm to bone density in older adults. However,the approximately 5% weight loss achieved in the study may be below the threshold typically associated with significant bone loss. “A consistent, though nonsignificant, trend toward lower BMD and higher turnover markers in the semaglutide group warrants further investigation,” researchers noted.

The study’s limitations, including its small sample size, short duration, lack of assessor blinding, and limited monitoring of diet and physical activity, highlight the need for larger and longer-term trials. Given the increasing use of GLP-1RAs, defining the long-term risk-benefit profile for bone health in older adults is crucial, especially as higher doses or prolonged use of these medications may have different effects on the skeletal system. A visual representation of the BMD changes in both groups over time would be beneficial.

These early findings offer a degree of reassurance, but underscore the importance of continued research to fully understand the complex interplay between weight loss medications, bone health, and the aging process.
