Malaria Progress Stalled by Drug Resistance and Funding Shortfalls, WHO Report Finds

Despite significant advancements in malaria prevention – including the rollout of new vaccines and tools – global progress against the disease is faltering, with an estimated 282 million cases and 610,000 deaths reported in 2024, according too the World Health Organization’s (WHO) annual World Malaria Report. The report underscores a growing threat from antimalarial drug resistance and a critical shortfall in funding, jeopardizing decades of gains.
New Tools Offer Hope, But Challenges Remain
The fight against malaria has seen notable successes in recent years. Dual-ingredient insecticide-treated nets and WHO-recommended vaccines are proving effective, helping to prevent an estimated 170 million cases and 1 million deaths in 2024. Since the WHO approved the world’s first malaria vaccines in 2021, 24 countries have incorporated them into their routine immunization programs. Moreover, seasonal malaria chemoprevention – providing preventative medication during peak transmission seasons – has expanded dramatically, reaching 54 million children in 2024, a substantial increase from approximately 0.2 million in 2012.
Progress towards a malaria-free world is now at risk. The report warns that setbacks in 2022 and 2023 have not been recovered, and the number of deaths remains stubbornly high. the convergence of multiple challenges – including drug resistance, biological threats, external factors, and funding constraints – threatens to roll back the progress we have made over the past two decades. Though, none of these challenges is insurmountable. With the leadership of the most-affected countries and targeted investment, the vision of a malaria-free world remains achievable.”
Drug Resistance and Biological Threats Complicate Efforts
The report highlights a particularly alarming trend: the growing resistance of malaria parasites to artemisinin derivatives, the cornerstone of malaria treatment following the failures of earlier drugs like chloroquine and sulfadoxine-pyrimethamine. Antimalarial drug resistance has been confirmed or suspected in at least eight African countries, with early indications that drugs combined with artemisinin are also losing efficacy.
Beyond drug resistance, othre biological factors are hindering progress. Malaria parasites exhibiting deletions in the pfhrp2 gene are widespread, compromising the accuracy of rapid diagnostic tests. Furthermore, pyrethroid resistance – reducing the effectiveness of insecticide-treated nets – has been confirmed in 48 countries. The emergence of Anopheles stephensi mosquitoes, resistant to many common insecticides, in nine African countries poses a serious threat to urban malaria control.
External Factors Exacerbate the Crisis
The challenges extend beyond biological factors. Extreme weather events, driven by climate change, are altering mosquito habitats and transmission patterns, leading to increased outbreaks. Conflict and instability in affected regions disrupt health services, limiting access to diagnosis and treatment.
Critically,global funding has plateaued over the past decade. In 2024, US$3.9 billion was invested in the malaria response, falling far short of the US$9.3 billion target set by the global Technical Strategy for malaria 2016-2030. Recent cuts in Official Progress Assistance (ODA) have weakened health systems,hindering surveillance efforts and forcing the cancellation of planned malaria surveys. These funding shortfalls heighten the risk of stock-outs and delays in crucial interventions.
New Medicines and Renewed Commitment are Crucial
Despite the grim outlook, there is cause for cautious optimism. The development of Ganaplacide-Lumefantrine, the first non-artemisinin combination therapy, represents a significant breakthrough in combating drug resistance. “The World Malaria Report is clear: drug resistance is advancing. Our response must be equally clear – new medicines with new mechanisms of action,” said a senior official from Medicines for malaria Venture.
Achieving a malaria-free future requires sustained political commitment,increased investment,and a unified global response. The WHO urges malaria-endemic countries to uphold the commitments outlined in the Yaoundé Declaration and to embrace the collaborative approach of the Big Push initiative to mitigate current and future threats.
