Musicians’ Hearing: Higher Risk of Tinnitus, Hearing Loss & Hyperacusis

Musicians face significantly higher rates of hearing-related issues – tinnitus, hearing loss, and hyperacusis – compared to their non-musician counterparts, according to a new comprehensive analysis. The findings, published in Otolaryngology–Head and Neck Surgery, the official journal of the American Academy of Otolaryngology–Head and Neck Surgery Foundation (AAO-HNSF), underscore the occupational risks inherent in a life dedicated to music. The study pooled data from 67 studies encompassing over 28,000 musicians across 21 countries, providing the most robust evidence to date on the topic of hearing health in the music community.

The research revealed a stark disparity: 42.6% of musicians reported experiencing tinnitus – the perception of ringing or buzzing in the ears – compared to just 13.2% of individuals in control groups. Hearing loss was also more prevalent among musicians, affecting 25.7% versus 11.6% in the general population. 37.3% of musicians reported hyperacusis, an increased sensitivity to everyday sounds, compared to 15.3% of non-musicians. These statistics highlight the vulnerability of those who regularly expose themselves to high sound levels, even if they aren’t consciously aware of the long-term consequences.

The Silent Struggle of Musicians

The impact of these conditions can be profound, affecting not only a musician’s ability to perform but also their overall quality of life. Tinnitus, in particular, can be debilitating, causing anxiety, sleep disturbances, and difficulty concentrating. Hearing loss can lead to social isolation and difficulty communicating. Hyperacusis can make everyday environments overwhelming and painful. Many musicians, according to Shaun A. Nguyen, MD, Professor in the Department of Otolaryngology–Head and Neck Surgery at the Medical University of South Carolina, “quietly live with ringing ears, sound sensitivity, or hearing loss, no matter whether they play in a symphony hall or a compact club.”

Dr. Nguyen emphasizes the limitations of current research, noting that much of it relies on self-reported symptoms. “Yet the research we have is still imperfect, often based on self-reported symptoms and missing details about things like other loud hobbies, specific instruments, and how consistently people leverage hearing protection. What we really need now is more personal, musician-entered risk profiling so we can offer practical, tailored advice that helps artists protect their hearing without sacrificing the music they love.”

The study also examined the nature of tinnitus among musicians, finding that the majority (76.3%) described their symptoms as occasional, whereas 15.6% reported permanent tinnitus. A significant portion – approximately 63% – of reported hearing loss cases were based on subjective self-report, with only 37% confirmed through objective audiometric testing. This suggests the actual prevalence of hearing loss among musicians may be even higher than currently estimated.

Genre Doesn’t Dictate Risk

Contrary to previous assumptions, the research found no significant difference in the prevalence of hearing loss, hyperacusis, or tinnitus between classical and pop/rock musicians. This challenges the notion that certain musical genres are inherently more damaging to hearing. The authors suggest that individual factors, such as the type of instrument played, seating position within an ensemble, room acoustics, and – crucially – attitudes toward hearing protection, likely play a more significant role in auditory risk than genre alone.

Understanding these individual risk factors is key to developing effective preventative strategies. Musicians who play louder instruments, sit closer to speakers, or perform in poorly soundproofed venues are at increased risk. However, even musicians who play quieter instruments or perform in more controlled environments can be vulnerable if they don’t take steps to protect their hearing.

What Can Musicians Do?

Protecting hearing requires a multi-faceted approach. The American Academy of Otolaryngology–Head and Neck Surgery offers resources and guidance on hearing conservation. Strategies include:

  • Using hearing protection: Custom-fitted earplugs or high-fidelity earplugs designed for musicians can reduce sound levels without significantly altering the sound quality.
  • Limiting exposure time: Taking breaks from loud environments can help reduce the cumulative impact of noise exposure.
  • Monitoring sound levels: Using sound level meters to measure the intensity of sound can help musicians identify potentially hazardous situations.
  • Regular hearing checkups: Annual audiometric testing can help detect early signs of hearing loss and allow for timely intervention.

The findings from this meta-analysis serve as a critical reminder of the importance of hearing health for musicians.
