The pursuit of a tan, once considered a sign of health and beauty, carries a significantly higher risk than many realize. New research published in the journal Science Advances reveals that indoor tanning nearly triples the risk of melanoma, the deadliest form of skin cancer and inflicts widespread DNA damage across almost the entire skin surface – damage far exceeding that caused by natural sunlight. This groundbreaking study, led by Northwestern Medicine and the University of California, San Francisco, maps the molecular consequences of tanning bed employ, challenging long-held industry claims about their safety.
Melanoma accounts for approximately 11,000 deaths annually in the United States, according to the American Cancer Society. For years, public health officials have cautioned against indoor tanning, but the precise mechanisms linking it to cancer remained unclear. The indoor tanning industry has often argued that their devices are no more dangerous than the sun, capitalizing on this knowledge gap. This new research, however, provides irrefutable evidence at the molecular level, demonstrating that tanning beds fundamentally alter skin cells in ways that surpass the impact of everyday sun exposure.
“Even in normal skin from indoor tanning patients, areas where there are no moles, we found DNA changes that are precursor mutations that predispose to melanoma,” explained Dr. Pedram Gerami, professor of skin cancer research at Northwestern University Feinberg School of Medicine and first author of the study. “That has never been shown before.” This discovery is particularly concerning because it suggests that even individuals without visible skin damage from tanning beds may be at increased risk of developing melanoma.
Uncovering the Genetic Scars of Indoor Tanning
The study involved a comprehensive analysis of medical records from roughly 6,000 individuals – 3,000 with a history of tanning bed use and 3,000 who had never used them. Researchers found that 5.1% of those who had tanned indoors were diagnosed with melanoma, compared to 2.1% in the control group. This translates to a 2.85-fold increase in melanoma risk, even after accounting for factors like age, sex, sunburn history, and family history of the disease. But the epidemiological findings were only the beginning.
To understand the biological impact of tanning beds, the team performed advanced genomic sequencing on melanocytes – the pigment-producing skin cells where melanoma originates – from 182 individual skin biopsies. These biopsies were collected from three groups: 11 patients with extensive tanning bed histories, nine individuals who had never tanned indoors, and six cadaver donors to provide a robust control group. The results were striking. Melanocytes from tanning bed users carried nearly twice as many genetic mutations as those from the control groups, and these mutations were frequently associated with melanoma development.
What’s more, these damaging mutations weren’t limited to areas of the skin typically exposed to the sun. They were found across almost the entire skin surface, even in regions usually covered by clothing, such as the lower back and buttocks. “In outdoor sun exposure, maybe 20% of your skin gets the most damage,” Dr. Gerami noted. “In tanning bed users, we saw those same dangerous mutations across almost the entire skin surface.” This widespread damage underscores the unique and pervasive threat posed by indoor tanning.
A Personal Toll: One Survivor’s Story
The research resonated deeply with Heidi Tarr, a 49-year-traditional melanoma survivor from the Chicago area who participated in the study by donating skin biopsies. As a teenager, Tarr frequently used tanning beds, driven by the prevailing beauty standards of the time. “It felt like that’s what made you beautiful,” she recalled, explaining that friends and celebrities were as well embracing indoor tanning.
Years later, in her thirties, Tarr discovered a mole on her back that turned out to be melanoma. This led to surgery and years of follow-up appointments, including more than 15 additional biopsies as new moles appeared. “The biopsies can be painful, but the mental anxiety is worse,” she shared. “You’re always waiting for the call that it’s melanoma again.” Tarr’s willingness to contribute to the research stemmed from a desire to help others avoid the same experience. “If what happened to my skin can help others understand the real risks of tanning beds, then it matters,” she said.
Calls for Stronger Regulations and Increased Awareness
The study’s findings have prompted calls for stricter regulations surrounding indoor tanning. Dr. Gerami believes that, at a minimum, indoor tanning should be illegal for minors. “Most of my patients started tanning when they were young, vulnerable and didn’t have the same level of knowledge and education they have as adults,” he said. “They feel wronged by the industry and regret the mistakes of their youth.”
He also advocates for warning labels on tanning beds that are comparable to those found on cigarette packages, explicitly stating the risk of cancer. The World Health Organization classifies tanning beds as a Group 1 carcinogen – the same category as smoking and asbestos. Dr. Gerami recommends that anyone who frequently used tanning beds in the past schedule a total-body skin examination with a dermatologist and discuss ongoing skin checks with their physician.
This research provides a crucial understanding of the long-term consequences of indoor tanning. It’s a stark reminder that the pursuit of a tan comes at a potentially devastating cost.
