For millions of people using GLP-1 receptor agonists to manage obesity and diabetes, the experience has been transformative. But for a significant minority, the results are stubbornly absent. Current data suggests that nearly one in four people do not respond to these treatments, losing little to no weight and seeing few of the cardiovascular or metabolic improvements associated with the drugs.
A new study suggests that how well GLP-1 weight loss drugs work may depend on your genetics, providing a biological explanation for why some patients thrive on the medication even as others notice no change. By analyzing the genetic profiles of nearly 28,000 individuals, researchers have identified specific variants that influence both the efficacy of the drugs and the severity of their side effects.
The research, published in Nature, utilized self-reported data from 23andMe users who had taken medications such as semaglutide (sold as Wegovy and Ozempic) or tirzepatide (sold as Zepbound and Mounjaro). The findings point to common quirks in the human genetic code that dictate how the body interacts with these powerful hormones.
The Role of the GLP1R Gene
The primary target of these medications is the GLP-1 receptor, a protein that tells the brain to feel full and the stomach to slow its emptying. The researchers identified a specific variant in the GLP1R gene—which encodes this receptor—known as rs10305420. This variant appears to act as a volume knob for the drug’s effectiveness.
The study found a clear correlation between the number of copies of this variant a person carries and the amount of weight they lose. Because humans typically have two copies of any given gene, the impact varies by dosage of the variant:
- One copy of rs10305420: Participants lost an average of about 1.7 pounds more than those without the variant.
- Two copies of rs10305420: Participants lost more than three pounds more than those without any copies.
While a few extra pounds may seem modest, obesity geneticists note that in the context of public health, these margins are significant. Ruth Loos, an obesity geneticist at the University of Copenhagen, noted that the total average weight loss for participants was approximately 25 pounds. Giles Yeo, an obesity geneticist at the University of Cambridge, emphasized that even a modest amount of weight loss—as little as 5 percent—can lead to meaningful health benefits, including lower cholesterol levels.
Adam Auton, study co-author and vice president of human genetics at the 23andMe Research Institute, explained that the variant likely improves the efficiency of transporting receptors to the cell surface. This increase in available receptors allows the medication to bind more effectively, enhancing the overall drug response.
The Trade-off: Efficacy vs. Side Effects
The same genetic “advantage” that leads to greater weight loss may also come with a cost. The rs10305420 variant was linked to a higher risk of gastrointestinal side effects, such as nausea, and vomiting. This creates a complex biological loop: the drug works better at a cellular level, but the resulting nausea can further suppress appetite, contributing to additional weight loss.
The research also highlighted a second variant, rs1800437, located in the GIPR gene. This variant specifically affected those taking tirzepatide, a “dual agonist” that targets both GLP-1 and gastric inhibitory polypeptide (GIP) receptors. Tirzepatide is generally known to cause less nausea than semaglutide because the GIP receptor is believed to act as a buffer against the nausea caused by GLP-1 activation.
Still, for people with the rs1800437 variant, this buffering effect is reduced. When combined with the first variant, the risk of severe side effects spikes. The study estimated that individuals with two copies of both rs1800437 and rs10305420 were 15 times more likely to experience vomiting while using tirzepatide than those without these variants.

Moving Toward Personalized Weight Loss
These findings provide a “proof of concept” for the future of precision medicine in obesity treatment. If clinicians could use a genetic screen to identify a patient’s variants before prescribing a drug, they could potentially select the medication most likely to work for that individual or adjust the dosage to mitigate side effects.

However, genetics are only one piece of the puzzle. The researchers cautioned that the rs10305420 variant is primarily present in about 40 percent of people with European and Middle Eastern ancestry, meaning the current data does not yet fully represent global populations. Other biological factors play a critical role in how a person responds to GLP-1 therapy, including:
- Age and Sex: Hormonal and metabolic differences across demographics affect drug metabolism.
- Comorbidities: The presence of Type 2 diabetes or other metabolic disorders can alter response rates.
- Drug Choice: Whether a patient is on a single-agonist (semaglutide) or dual-agonist (tirzepatide) changes the receptor interaction.
As Giles Yeo summarized, “Your genes do matter, but it’s not only your genes.” Rare genetic variants not captured in this large-scale analysis may also exert a significant influence on weight-loss variations.
