SKKU Researchers Develop World’s First Oral Microbiome Nanomedicine

Researchers from Asiaresearchnews and the University of Michigan have developed what is described as the world’s first oral, microbiome-based nanomedicine designed to supercharge anti-cancer immunity. The international collaborative study was published in Nature Nanotechnology, a premier journal in nanoscience and medicine.

World’s First Oral Microbiome Nanomedicine Developed

The research team was led by Professor Young Seok Cho of the Department of Medicine and School of Medicine at Sungkyunkwan University—who specializes in Nuclear Medicine and Molecular Imaging—alongside Professor James J. Moon’s team at the University of Michigan.

Overcoming the Limitations of Immunotherapy

While immunotherapies such as anti-PD-1 drugs have emerged as revolutionary treatments that reawaken the body’s own immune system to fight cancer, their effectiveness has faced clinical challenges because only a small fraction of patients respond positively.

To address low response rates, the scientists focused on the connection between the gut microbiome and human immunity. They identified a naturally occurring gut microbial metabolite known as 3,4-dihydroxybenzoic acid (DHB). According to the findings, DHB acts as a powerful immune booster that safely drives defenses against tumors by promoting stem cell-like properties in CD8+ T cells, which are critical white blood cells responsible for destroying cancer.

Ordinarily, T cells suffer metabolic exhaustion and lose their fighting capability due to an overactive sugar breakdown process known as glycolysis. DHB suppresses this glycolytic process, effectively flipping on a survival switch that helps CD8+ T cells resist exhaustion, survive for longer periods, and continuously sustain their attack on cancer cells.

Applying Nanotechnology to Extend Bioavailability

A major obstacle previously prevented natural DHB from being used directly as a therapeutic drug: once ingested, it breaks down rapidly and is cleared from the body within just a few minutes.

To overcome this limitation, the research team applied advanced nanotechnology. They chemically synthesized a derivative modifying the structure of DHB and designed an oral nano-emulsion formulation resembling olive oil—named Prodrug 201—by precisely mixing oil and water. This stable oral nanomedicine drastically extended the time the active substance stays in the body, boosting its overall absorption rate, or bioavailability, by 14.3 times compared to the natural substance.

Animal Studies Show Complete Tumor Eradication

In rigorous animal studies involving test models bearing various cancers, including colorectal cancer, melanoma, and breast cancer, the administration of Prodrug 201 directed large numbers of refreshed, stem-like T cells directly into tumor sites, resulting in dramatic tumor shrinkage.

When the oral nanomedicine was administered alongside existing immune checkpoint blockade therapy, it completely eradicated tumors in test models and generated long-term immune memory capable of preventing cancer recurrence.

This study unveils the molecular secret of how gut microbial metabolite regulate our immune system and successfully translates that knowledge into a convenient, oral medication through nanotechnology, said Professor Young Seok Cho, who led the research, as reported by AlphaGalileo.

By converting previously untapped microbial resources into a safe and scalable nanomedicine, this work builds a critical foundation for commercializing next-generation microbiome-based cancer immunotherapies, Cho added.
