If you or your loved one is diagnosed with Parkinson’s, a lot can be thrown at you. Organizing care can become complicated due to the varied symptoms and the rapidly changing clinical picture. It can then be nice if someone thinks along with you about your situation. For this reason, we have set up the Parkinson care path at Respect. During the care path, you and one or more specialized practitioners will look for the right care for you. Accessible care where you are in control.
Intake, Treatment, Aftercare
At the start of the care path you will receive a questionnaire about your daily life and the complaints you experience. On the basis of this, an intake interview follows. We use the questionnaire and the intake interview to map out your situation, your care need(s) and your wishes. Based on this information, the expertise team will draw up a personal care plan and advice will be formulated. You discuss the care plan and the advice together with the coordinating practitioner of your choice. You then give your own interpretation. A major advantage of the Parkinson’s care path is that communication during the care path mainly takes place via the coordinating practitioner. This way, your personal preferences can be better taken into account, such as the amount of information you want.
Every six months you evaluate your situations and your wishes together with the coordinating practitioner. If necessary, the care plan can be adjusted.
Even if you no longer receive treatments, the six-monthly evaluations with the coordinating practitioner will continue. Parkinson’s is different for everyone and the need for care can change quickly. The six-monthly evaluations enable us to respond to this as well and as quickly as possible.
For whom?
The care path is intended for people living at home with Parkinson’s who are being treated by a primary care provider. You can register with a referral from your healthcare provider.
