The constant ambition of Klalit to optimize the response to patients and maintain the continuity of care between the hospital and the community gave birth to a joint seminar for the nursing management of the Klalit in the Haifa and Western Galilee district and the nursing management of the Carmel Hospital

As part of the implementation of the processes designed to optimize the response to patients who are being treated in the community and in the hospitals, Rinat Bassel, the nursing director of Klalit in the Haifa and Western Galilee district, and Ahova Tal, the head nurse of the Carmel Hospital, organized a day of familiarization and work on the hospital-community care continuum.

The goal of the day was to create joint work processes, in order to maintain a therapeutic continuum and ensure an easy transition for the patient upon his release from hospitalization to the continuation of treatment within the community.

80 brothers and sisters from the hospital and the community were present at the study day, among them the nursing management in the district, nursing directors of clinics, the nursing management of Carmel Hospital and responsible brothers and sisters at the hospital. During the meeting, the participants gathered in work groups, after which they presented 10 products of new care continuum processes that will be developed during the year for joint work processes, thinking about placing the patient at the center.

Also presented were the two processes of the continuum of care that are already being carried out optimally today: discharge of a patient with a severe healing wound and discharge of women at risk of postpartum depression. One of today’s products is the creation of a treatment sequence process for women who have experienced pregnancy loss up to the 22nd week.

“On this day we learned to get to know the professionals behind the phone and the email and to work in full partnership to safely transfer the patient to the clinic in the community and to know that he will be received by professional hands,” said Ahuva Tal, the head nurse of Carmel Hospital.

Rinat Besel, the district nursing director of Klalit in the Haifa and Western Galilee district, concluded: “This important day was created with the aim of strengthening the relationship between the nursing teams at the hospital and the community in order to enable optimal and continuous care for patients who need it. I am happy that we were able to deepen the relationship and develop new work processes between the teams”.
