Abdominal pain and fluid in the small pelvis

A 58-year-old woman comes to our Gynecology outpatient clinic because of abdominal pain and fluid in the small pelvis, which was seen on an abdominal ultrasound. Her history includes laparoscopic sterilization using Fallope rings.

Transvaginal ultrasound shows a multilocular abnormality of 5 by 3.5 cm that is completely transonic (see picture 1, top left) and is located next to the right ovary. We are thinking of a hydrosalpinx or paraovarian/tubal cyst. PCR of the cervix, infection lab and CA-125 are all without detail. CT abdomen describes a fluid collection of 8×3 cm in the Douglas cavity with no further intra-abdominal abnormalities.
