Hoffmann-La Roche Limited announced that Health Canada granted market authorization for the clot-dissolving agent TNKase, known generically as tenecteplase, with the approval taking effect effective November 13, 2025. The decision represents the first new stroke treatment authorized in nearly 27 years.
Pivotal Trial Evidence Behind the Canadian Authorization
The regulatory authorization rests upon data generated by the AcT trial, formally titled Alteplase Compared to Tenecteplase in Patients With Acute Ischemic Stroke. Led through the University of Calgary with financial support from the Canadian Institutes of Health Research and the Hotchkiss Brain Institute, the multicenter, open-label, parallel-group, randomized controlled trial enrolled patients across 22 primary and comprehensive stroke centers in Canada. Investigators evaluated adults presenting within 4.5 hours of symptom onset who experienced disabling neurological deficits from ischemic stroke, assigning participants on a 1:1 basis to receive either intravenous tenecteplase or the standard-of-care agent alteplase.

Following those trial results, tenecteplase has been integrated into the Canadian Stroke Best Practices recommendations as an alternative option for intravenous thrombolysis in eligible patients. Roche Canada plans to introduce a new 25 mg vial configuration in the coming months to facilitate administration.

The American Stroke Association, a division of the American Heart Association, published its 2026 Guideline for the Early Management of Patients With Acute Ischemic Stroke in the journal Stroke, replacing the previous 2018 edition and 2019 update. The new framework introduces first-time guidance dedicated to pediatric stroke, addressing infants, children, and teens.
