Patients with severe heart failure can now also be treated at home through a collaboration between the Leiden University Medical Center (LUMC), Marente and Zorggroep Katwijk. With this new specialized home treatment, changes in heart failure can be detected and treated at an early stage.

By using a nurse specialist and technology, patients can be treated at home and hospitalization can be avoided. The home treatment was made possible in part by health insurer Zorg en Zekerheid.

With heart failure, most people experience physical complaints such as fluid in the legs, shortness of breath and fatigue. When patients retain too much fluid, they are given diuretics in tablet form or via an IV. Until recently, the treatment via an infusion took place in the hospital, but can now also be done at the patient’s home. For some time now, the Heart Diseases Department of the LUMC, care organization Marente and Zorggroep Katwijk (general practitioners) have been working closely together to give shape to intensive care for patients with severe heart failure at home.

The healthcare parties are therefore pleased that this is now possible. In this way, patients can recover at home in their own familiar environment with the same quality of care as in the hospital. The cardiologist or general practitioner and a cardiology nurse specialist from Marente assist patients with this. The treatment will initially be offered in the municipalities of Katwijk and Leiden.

Promotion of quality of life
The first experiences are positive. ‘It’s fantastic that I can receive this care at home in my own safe environment’, says one of the first patients. Telemonitoring plays a role in heart failure treatment at home. Patients receive a package with various devices for taking home measurements; Called ‘The Box’.

With a blood pressure monitor, scales and an app, a patient can closely monitor his or her own health and the healthcare provider is alerted in time if necessary. The healthcare providers expect that patients with heart failure will be able to receive the right treatment in their own home situation for longer with the new specialized home treatment. As a result, the quality of life will improve and the number of patients in the emergency room and/or hospital admissions will decrease.
