New studies reveal that high blood pressure quietly damages the heart decades before symptoms appear. Researchers discovered that nearly one in ten over-50s in high-risk groups suffered unrecognised heart attacks, while lifelong blood pressure tracking shows that even slightly elevated readings in early adulthood steadily reduce heart blood flow later in life.
Hidden Heart Attacks Discovered Across Manchester Communities
A snapshot of an ongoing study funded by the British Heart Foundation and Innovate UK has uncovered a shocking proportion of undetected cardiac events among older adults. Researchers from the University of Manchester and Manchester University NHS Foundation Trust examined heart scans from 550 volunteers aged over 50 who presented with common cardiometabolic issues. Nearly one in ten participants—representing nine per cent of the group—had already suffered a heart attack without ever realising it.
To qualify for the Detecting EARLY Heart Failure in Greater Manchester (EARLY-HF) study, volunteers needed at least two conditions from a list of seven prevalent health issues, including type 2 diabetes, chronic kidney disease, and chronic obstructive pulmonary disease. Pre-existing high blood pressure was diagnosed in 88 per cent of the study group, while 70 per cent experienced obesity and 42 per cent lived with diabetes.
Led by Professor Chris Miller, researchers found that almost two-thirds of the study group—60 per cent—had high blood pressure that remained improperly controlled or unmanaged. Dr Nicholas Black, a British Heart Foundation Research Fellow who presented the findings at the British Cardiovascular Society conference in Manchester, noted that the team did not anticipate finding so many unrecognised cases of heart disease.

While our study involves people who would be considered at higher risk of cardiovascular disease, finding so many people who had experienced a heart attack and didn’t know it was not something we ever anticipated going into this study.

The data showed that every 10-point rise in systolic blood pressure between ages 36 and 69 reduced heart blood flow by up to 6 per cent by age 77. Furthermore, blood pressure increases between ages 43 and 63 reduced heart blood flow by up to 12 per cent. The study noted that every 1 per cent reduction in blood flow corresponds to a 3 per cent higher risk of experiencing a major cardiac event.
