Prior Authorization Streamlining

Will insurers follow through on their pledge to streamline prior authorization? Skepticism remains, with many believing that changes will primarily benefit the insurers themselves. The expectation is that FHIR will be used for high-volume, inexpensive items to quickly reduce labor costs.

Complex but urgent items will likely still require review by clinicians, whose qualifications may be limited and whose primary role may be to deny requests. Policies for patient transitions between insurers are expected to have numerous exceptions.

In 2018, insurers pledged to reduce prior authorization (PA) requirements for providers with good performance and adherence to evidence-based medicine, and automate the process using national standards.

The Centers for Medicare & Medicaid Services (CMS) issued a rule in January 2024, mandating payers to publish a prior authorization API by January 1, 2027. Insurers will have three days to respond to urgent requests and seven calendar days for standard requests.
