Visiting a urologist for regular check-ups is crucial for men’s health, yet many seem to avoid it due to fear and other reasons, according to a study by Plan International. The study revealed that approximately half of the men surveyed between the ages of 18 and 35 ignored health problems. Neglecting these check-ups can have serious consequences, especially in the field of urology.

Dr. Christoph Pies, a renowned urologist and author of “Don’t be afraid of the urologist,” addressed this issue in an interview. He explained that fear of a bad diagnosis and the resulting consequences often deter men from seeking medical attention. The notion of illness representing weakness contradicts the traditional role of being a strong man, leading to the development of avoidance strategies.

One of the primary reasons men avoid visiting urologists is the lack of time, followed by fear of bad news and hesitation towards the prostate examination involving rectal palpation. Consequently, men attend health check-ups significantly less frequently than women, with a provision rate of just over 20% as compared to over 40% for women.

Dr. Pies emphasized the importance of routine urological examinations and outlined the components of a standard examination. This involves palpation of the testicles and groin area, inspection of the genital region for inflammation and skin changes, assessment of the urethral opening, and the prostate examination (from age 45) through palpation with a finger from the anus.

Regular check-ups play a crucial role in early detection of prostate tumors. Statistics show that only one out of ten existing prostate tumors is discovered, usually those on the outside of the prostate. Currently, the most reliable method to gain insight into the prostate’s internal status is through the prostate-specific antigen (PSA) blood test, which is self-financed.

Dr. Pies stressed the significance of timely check-ups and the examinations that shouldn’t be missed. These include vaccination against human papilloma viruses (HPV) for boys aged 9 to 17, testicle self-examination for men over 20, regular health check-ups at the family doctor from the age of 35, and prostate examinations starting at 45 or earlier for those with a family history of prostate cancer.

Other vital check-ups include colon cancer screening from age 50, flu and pneumococcal vaccinations from age 60, and an ultrasound to check for abdominal aorta enlargement at 65. Additional examinations, such as the PSA test for prostate cancer, may be recommended based on individual cases.

Dr. Pies lamented that many men only visit a doctor when experiencing acute symptoms. Typical symptoms that prompt men to seek urological attention include prostate inflammation, urinary problems due to an enlarged prostate, anxiety about sexually transmitted diseases and concerns about testicular cancer.

He noted a generational divide where older men tend to ignore signs of illness unless something is significantly wrong, while younger men are increasingly health-conscious. However, excessive hygiene among the younger population leads to conditions such as “overtreatment balanitis”—an inflammation of the glans caused by excessive washing with soap or shower gel.

Dr. Pies frequently encounters questions from patients about potential illnesses and concerns surrounding testicular and prostate cancer. Additionally, young men often express concerns about “normal sexual function,” including penis size and appearance. Addressing these concerns through open and trusting conversations is vital for patients to understand their conditions and find appropriate treatment.

Regular check-ups, along with open communication and awareness, are essential for maintaining and promoting men’s health. By overcoming fears and societal expectations, men can prioritize their well-being and reap the benefits of early detection and timely treatment.
