There are two interpretations.
The first is the one that it offers, that is, it would be the descriptive name of what the sculpture represents, and in this case it goes in round and lower case: “the lady of Elche”. This form has the advantage that it allows mentioning the two works without problems: “the lady of Elche and the lady of Baza” or “the ladies of Elche and Baza”.
But it is also possible to consider, as clarified in the account of X of the RAEwhich, even without actually being a title given by the author, functions as such, so it is then Lady of Elchein italics and with initial capital letter.
