A 45-year-old construction worker arrived at the emergency department with a swollen, excruciatingly painful right shoulder. Initial X-rays revealed an extraordinary absence: his entire shoulder joint had been destroyed, with the humeral head—normally the “ball” of the ball-and-socket joint—completely missing. The upper arm bone (humerus) floated freely, unattached to the shoulder blade, while the rotator cuff was also severely damaged. This case, documented in the New England Journal of Medicine, describes a rare condition known as Milwaukee Shoulder Syndrome
(MSS), first identified in 1981 among elderly women in Wisconsin.
A Rare Condition with Severe Consequences
MSS, a subtype of rapid destructive arthritis, is characterized by the complete loss of the humeral head and extensive rotator cuff tears. The condition’s rarity has led to delayed diagnoses, as seen in the case of Vance Courtney, a Georgia IT worker who initially received a hopeless prognosis for his shoulder pain. Originally, Courtney was told there was nothing that could be done for it.
But he found it and diagnosed it.
The syndrome’s origins trace back to 1981, when four elderly Wisconsin women presented with similar joint destruction. A 1982 study later linked MSS to rapid destructive arthritis, a broader category of degenerative joint disease.
Innovative Surgery Offers Hope
He explained everything for me, all the possible negatives that could happen,
Courtney said. When I started having issues with the healing process, he got me some things to work on, and I got back to the normal healing schedule.
Another patient, Sgt. Andrew Morris, faced a similar challenge. A soldier based at Fort Stewart, Georgia, Morris had recurrent shoulder dislocations that wore away the front of his socket. Parada performed a procedure involving a donor ankle bone and cartilage to rebuild the damaged area. The surgery we do to take care of that is a really cool surgery that one of my buddies who was also a military orthopedic surgeon in the Navy invented, and he and I have done a lot of research on perfecting this surgery,
Parada said. It’s not a very common surgery right now, and part of that is because it’s a very challenging surgery.
Both patients experienced significant recovery. Courtney resumed jet-skiing and other physical activities, while Morris, who had previously been told his condition was untreatable, reported nearing 100% functionality. I won’t be back to 100 percent, but I’ll be pretty danged close to it,
Morris said.
The Role of Advanced Imaging and Expertise
Accurate diagnosis of MSS and similar conditions often hinges on advanced imaging techniques like MRI, which can reveal subtle damage to ligaments and cartilage. Without the correct type of the MRI, the injury usually goes unnoticed because it is so rare,
Parada explained.
