From 1st October, Dr. Alice Costamagna will be joining the single role of Primary Care with a elective cycle (currently self-limited to a thousand patients until the achievement of the specific training diploma in General Medicine) as a doctor. She will provide her services in Mondovì, at the main clinic in via Quadrone, 8 (telephone for emergencies 353/4736822; tel. for prescriptions and appointments 0174/552558 – Email: [email protected])
The doctor will receive by appointment on Monday, Tuesday and Friday from 10 am to 12 pm; Wednesday and Thursday from 6:30 pm to 7:30 pm.
Patients who wish to choose a doctor can do so online, if they have the credentials to access the Piedmont System at the website, otherwise they can go, with their health card, tax code, and proxy (in the event that the person concerned cannot appear), accompanied by a photocopy of their identity document, to the Choice/Revocation Desks at the Mondovì hospital in via San Rocchetto 99 (Monday to Friday from 8 am to 5 pm).
Patients will be able to make the new choice of Doctor within the single territorial area of the District of residence/domicile, with the limitation that if the distance between their residence/domicile and the office of the general practitioner they intend to choose exceeds 20 kilometers, they will have to have the acceptance by the doctor himself, compatible with the availability of places at the time of the choice.
