Psoriatic Arthritis and the Temporomandibular Joint: Symptoms and Treatment

A systematic review of 27 studies and 151 patients summarized the study situation on psoriatic arthritis (PsA) involving the temporomandibular joint. The studies showed a wide range of symptoms for which a temporomandibular joint problem should be considered in the context of PsA. Conservative therapy typically achieved significant symptom and functional improvements. Psoriasis is a chronic inflammatory autoimmune disease that affects the skin, but also often affects the joints. When you think of this so-called psoriatic arthritis (PsA), you primarily think of affected finger joints or knees, among other things. The present systematic review examined reports of PsA involving the temporomandibular joint, i.e. the temporomandibular joint, which symptoms can occur and how they are treated. What symptoms occur when psoriatic arthritis involves the temporomandibular joint? The authors identified relevant studies from the medical-scientific databases PubMed, Web of Science, Embase, Scopus and Ovid and analyzed the results and quality of the studies. The analysis included 23 publications, including 10 case reports. A total of 151 patients with temporomandibular joint problems in PsA were reported. The patients had suffered from psoriasis for between 1.5 and 24 years. Clinical symptoms of temporomandibular joint involvement included: Jaw joint pain Jaw joint noises (e.g. clicking or cracking) Restricted joint mobility Swelling in front of/below the ear (preauricular) Misaligned teeth Headache Tinnitus Stiff neck Nutritional restrictions TMJ involvement was examined in 6 studies using magnetic resonance imaging (MRI). CT (computed tomography) was used in 8 studies. Ultrasound examinations were carried out in 2 studies. Treatment of TMJ PsA symptoms was performed topically and systemically in 11 studies. In 5 studies, surgical measures were also necessary to improve symptoms of jaw joint stiffening (ankylosis) and joint function. Conservative therapy usually improves symptoms significantly The analysis of 23 studies showed a wide range of symptoms in cases of PsA involving the temporomandibular joint. However, temporomandibular PsA did not appear to be different from other forms of arthritis of the temporomandibular joint radiographically and clinically and should also be considered as a possible symptom in PsA with a number of accompanying symptoms.
