Psychodynamic therapy and cognitive therapy are now widely used in Italy for the treatment of various psychic disorders, such as depression, anxiety states, obsessive-compulsive disorder, but also for other states of psychological suffering. They are based on different basic concepts, so much so that they are often considered to be in opposition. But it is increasingly evident that all forms of psychotherapy also have a lot in common. There psychodynamic therapy a psychotherapy derived from psychoanalysis. In Italy psychoanalysis began to spread widely, especially starting from the 1960s. The aim of psychoanalysis is not only the direct resolution of symptoms, but also an examination of the patient’s overall way of functioning and of his intrapsychic phenomena. It gives importance above all to unconscious phenomena and uses the relationship between therapist and patient as a fundamental tool for treatment, through the analysis of the so-called transference.
In classical psychoanalysis the patient lies on the couch and the therapist sits behind him, three to five sessions per week are carried out and the treatment can last for many years. In reality, contemporary psychoanalysis has increasingly shifted attention from the examination of the internal world to that of interpersonal relationships. After Freud, it fragmented into various directions, so much so that today psychoanalysis no longer exists, but there are many psychoanalysts, numerous currents that are difficult to integrate into a single discipline. There psychodynamic therapy, as already said, a brief psychotherapy derived from psychoanalysis, but it can also be carried out with a single weekly session or a reduced number of weekly sessions. The patient is usually seated and not lying on the classic Freudian couch. Therapy aims at understand the unconscious reasons for which the patient’s psychological disorders or sufferings have developed. The therapist encourages him to express emotions and fantasies, even if they are disturbing, and tries to bring out any unwittingly repetitive behaviors, which can lead to unfavorable life choices. It also explores the patient’s early experiences that may be reliving unconsciously in current behaviors. It can therefore be said that psychodynamic therapy is similar to psychoanalysis with the difference that the couch is not used and there is a low weekly frequency of sessions.
It is based on different assumptions cognitive therapy, which spread in Italy starting from the seventies. Originally inspired by cognitive psychology, from which the name derives, it has changed over time following the developments in the sciences of the mind, in particular cognitive sciences and affective neuroscience. Initially he gave a lot of importance to thoughts, but later he shifted attention to emotions and the body, considered central aspects of psychic suffering. What remained stable the reference to the scientific method, therefore with a constant evolution to the evolution of the sciences of the mind. Cognitivist therapists are always attentive to the present moment, to what happens today in the patient’s life, with respect to the historical reconstruction, typical of psychoanalysis. They use specific techniques aimed at modifying the patient’s current behavior, techniques whose effectiveness has been confirmed by empirical studies. The sessions serve to free the patient from cognitive, emotional and relational patterns that condition their existence, then everything that is understood together with the therapist must be applied in real life, the real test.
