With the data about your health, your IBD nurse will advise you remotely. Remote care is an aid to treatment.

Remote care is an extra way to monitor your health yourself. This will give you more insight into your illness. You know when you are doing well or not, and what you can do yourself

Why remote care?

The risk with IBD is having a flare-up of your bowel disease. You will then become sicker. Sometimes you have to be admitted to hospital for this. The symptoms are different for everyone. You may experience abdominal pain, diarrhea and blood in your stools. You also get very tired very quickly. If you recognize your complaints in time, you can also tell your doctor in time. This way you can prevent a flare-up or even hospitalization. Remote care helps you with this. You gain more insight into your illness and know when you are doing well or not, and what you can do yourself.

For remote care, use the Thuismeten app and the BeterDichtbij app.

With the Thuismeten app you fill in a questionnaire at fixed times about your intestinal complaints, about how you feel and whether you have other problems due to your intestinal disease. You do this more often in a troubled phase of your bowel disease than in a calm phase. You can also use the app to pass on the results of the calprotectin test if this is necessary for your treatment. The IBD nurse will discuss with you whether you should have calprotectin tests.

A schedule will be prepared for you in the Thuismeten app. The app gives you a reminder. This makes it clear to you when you need to complete a questionnaire or do a calprotectin test.

The IBD nurses receive your data via the Thuismeten app. You will always receive a response when you send in a questionnaire or the results of a calprotectin test. Does the IBD nurse see that you are getting worse? Or that there is a risk of a flare-up of your disease? Then she will contact you personally via the BeterDichtbij app or she will call you. She will discuss with you which care suits you best at that moment.

Do you doubt yourself? You can always fill in an extra questionnaire yourself in the Thuismeten app. Or contact your IBD nurse via the BeterDichtbij app.

How do I measure my calprotectin level?

You only do calprotectin tests if your IBD nurse has discussed this with you. You measure your calprotectin value with the QuantOn cal home test. The IBD nurse will explain how to do the home test. And what you need for that.

Do you need new home tests? Then you can send a message via the BeterDichtbij app. You will then be sent new home tests.

How do I install the Home Measurement app?

To use the Thuismeten app, you need a smartphone or tablet. You must download the Home Measurement app from Luscii on your smartphone or tablet. Below is a step-by-step guide on how to do that.

  • You will receive an e-mail from Luscii about the Home Measurement app at the e-mail address you provided. You can then install the Home Measurement app. It is important that you can read this e-mail in the hospital so that we can explain the Thuismeten app to you in the hospital.
  • If you do not see the e-mail immediately, please check the spam or unwanted e-mail.
  • You download the Home Measurement app on your phone or tablet via the App Store or via the Play Store.
  • To complete the registration, you will receive a verification code by email. Enter this code within 15 minutes, otherwise the code will expire and you will have to register again.
