Rijn en Duin receives subsidy for ‘Heart for Connecting’ project to improve patient care

Regional organization Rijn en Duin in South Holland North has been awarded a subsidy from the National Health Care Institute for their project ‘Heart for Connecting’. The aim of the project is to empower patients with limited health skills, such as those suffering from diabetes or cardiovascular disease, to make informed decisions about their own care. This collaborative initiative involves 18 general practices, four primary care pharmacies, one hospital, and Zorg en Zekerheid, among other partners.

Zorg en Zekerheid has reported that Heart for Connecting is being carried out in partnership with Rijn en Duin and the National eHealth Living Lab (NeLL). The project actively engages individuals with varying degrees of health literacy, providing them with tailored medical information and utilizing digital tools such as scales and blood pressure monitors. The objective is to enhance patients’ understanding of medication usage, promote adherence, and encourage healthier lifestyles.

Targeting individuals with cardiometabolic disorders, including those at high risk of cardiovascular disease, diabetes, kidney disease, hypertension, and obesity, Heart for Connecting involves collaborations with renowned organizations such as the Health Base Foundation, Pharos, Nivel, Zorg en Zekerheid, and the Leiden University Medical Center (LUMC).

The project incorporates four scientifically proven interventions, in conjunction with the participation of eighteen general practices, four primary care pharmacies, and one hospital. These interventions include the implementation of a feedback method in pharmacies to ensure healthcare providers deliver information that is understandable to patients regarding medication usage. Patients themselves are involved in measuring risk factors such as blood pressure and weight. Additionally, patients are being educated about a healthy lifestyle, and compliance is being promoted through the use of clear and accessible labels.

This initiative aims to improve healthcare outcomes and empower patients with the knowledge and tools they need to actively participate in their own care. By bringing together various healthcare providers and organizations, Heart for Connecting seeks to create a collaborative environment that supports and enhances patients’ understanding of their health and their ability to make informed decisions.
