To request the activation of the coverage, the applicant must submit the following documents (original or notarial reproduction certification, before legalized copy) at any of our agencies:
Bag Content Theft:
Document proving the pre-existence of the stolen asset or assets (corresponding receipt or proof of payment, photographs of the asset, etc.).
Certified copy of the Police Report, which must have been filed within twenty-four (24) hours following the occurrence of the incident.
Affidavit presented to the COMPANY, through which the INSURED describes the circumstances of the accident and details of the stolen items.
Interview with the INSURED, whenever requested by the Company.
Copy of ID of the insured.
Death Certificate or Certificate, issued by RENIEC.
Complete Medical Certificate of Death, issued by MINSA.
Certified Copy of the Certificate or Full Police Report issued by the National Police of Peru, as appropriate.
Complete Clinical or Medical Necropsy Protocol, when applicable.
Result of Ethyl Dosage and/or Result of Toxicological Analysis, as appropriate.
Identity document of the Legal Heirs.
Literal copy of the final registration of the Will or Intestate Succession issued by the National Superintendent of Public Registries, which designates the Legal Heirs.
Expenses for Processing Personal Documents due to Robbery, Theft, Kidnapping or Loss:
Copy of ID of the INSURED.
Sealed copy or certified copy of the proceedings of the Public Ministry (complaint).
Proof of payment directly related to the issuance of duplicates of Personal Documents.
Affidavit of not having contracted or requested or processed coverage of any Insurance Policy that has coverage of the same nature.
