# Pilot Fernando Tapia Dies in San Felipe Plane Crash; Mechanical Failure Suspected
An examination is underway following a fatal plane crash in San Felipe, Chile, on Wednesday afternoon that claimed the life of the sole occupant, pilot Fernando Tapia. Preliminary reports suggest a mechanical failure may have been the cause of the accident.
The crash occurred in the May 21 sector of San Felipe, prompting a swift response from the General Directorate of Civil Aeronautics (DGAC) and the Valparaíso Investigative Police (PDI) Homicide Brigade. Investigation Launched, Focus on Mechanical Issues

The DGAC dispatched a team to the crash site to begin a thorough investigation. Claudio Pandolfi, Director of accident Investigation at the DGAC, acknowledged the complexity of the process, stating that officials are working diligently to produce a clarifying report “quickly.” He emphasized the importance of identifying both the causes and contributing factors to prevent similar tragedies in the future.
A senior official stated, “With this facts it will be possible to determine, as stated in our legislation, the causes and contributing factors that allow us to feed the aeronautical system to prevent these things from happening again.”
Official Statements Point to Mechanical Failure
Regional presidential delegate of Valparaíso, Yanino Riquelme, echoed the initial assessment, indicating that preliminary information points toward a mechanical issue.
“There is still no official or more advanced information in this regard,” Riquelme said. “The DGAC has been established, support has been requested from the PDI Homicide Brigade. Apparently, by the first signs there is mechanical failure.”
A prosecutor on duty from San Felipe was also present at the scene, awaiting assignment of the case by the Public Ministry.
