Specialists from the Monte Sinai Hospital in Guayaquil saved the life of a newborn – Ministry of Public Health

“From the health center they transferred me to Mount Sinai Hospital. The ultrasounds showed that my baby’s intestines were out of her place. So they had to schedule me for a C-section at 34 weeks into my pregnancy,” said patient Esther E.

A multidisciplinary team from the Mount Sinai General Hospital (HGMS), made up of neonatologists, gynecologists, anesthesiologists and pediatric surgeons, successfully performed the first intrapartum surgery at this Ministry of Public Health (MSP) establishment. The patient was a neonate diagnosed with fetal gastroschisis, that is, a malformation that affects 1 in 2,500 newborns.

To correct this defect that causes one or some of the baby’s organs to come out of his body, a highly complex surgery called Simil-Exit was performed through a hole in the abdominal wall. This surgical technique consists of performing a caesarean section to operate on the baby while he continues to be attached to his mother through the umbilical cord and placenta.

The specialists have a maximum of seven minutes to carry out the entire procedure, since during this time the circulation of oxygen to the baby through the placenta is maintained.

According to experts, this technique increases the chance of survival of the neonate compared to a traditional procedure. In addition, it reduces the risk of infections, since the intervention is given in a sterile environment, avoids the use of anesthetics on the patient and reduces intubation time and hospital stay.

For a neonate to be selected for surgery of this type, it must meet specific clinical conditions such as: adequate lung maturation, average weight, also that the abdominal wall has the ideal diameter to reintroduce the organs and that these do not show signs of inflammation.
