The new overweight and obesity guideline has been praised for its role in tackling the growing problem of obesity in the Netherlands. However, one bariatric surgeon is urging for more focus on the most effective approach for obese individuals.

According to recent figures, half of the adult Dutch population is overweight, with 15% of them classified as obese. Recognizing the serious consequences this poses for public health, the new guideline aims to provide healthcare providers with a framework for assessing and treating overweight and obesity.

Bariatric surgeon Marloes Emous, from Centrum Obesitas Noord-Nederland, commends the integrated guideline for its coverage of the entire range of overweight. She emphasizes the importance of appropriate treatment for overweight individuals and acknowledges that Combined Lifestyle Interventions (GLI) can be effective for those with a BMI of 25 to 30. These lifestyle programs target sleep rhythm, nutrition, exercise, and psychological well-being, lasting for at least a year.

However, Emous raises concerns about the effectiveness of GLI for obese individuals. She argues that the most effective treatment for this group is stomach reduction surgery combined with lifestyle changes. While the new guideline does mention surgical treatment as an option, she believes it is somewhat overlooked. Emous suggests that for individuals with a BMI of 35 or higher, going through a lengthy process before eventually resorting to surgery can be counterproductive.

Obesity, especially in individuals with a BMI of 40 or higher, has multiple causes, including lifestyle, hormones, stress, metabolism, and genetics. Given the serious health problems associated with obesity, Emous emphasizes the need for appropriate and effective treatment for this degree of overweight.

At Centrum Obesitas Noord-Nederland, where Emous works, gastric reduction surgery is the main treatment offered. However, patients are required to follow a lifestyle program before and after the operation, which lasts for up to one and a half years. Results in terms of sustainable weight loss have been positive, and patients are followed up for up to five years after surgery.

While the new guideline is a significant step towards addressing overweight and obesity, Emous urges for more recognition of the most effective approach for obese individuals. She emphasizes the importance of considering surgical treatment earlier in the process, rather than prolonging the journey with potentially less effective interventions.
