Temporary employment among nurses, a phenomenon that worries hospitals

In hospitals, the vicious circle of temporary work is not limited to the world of doctors. In the nursing ranks too, this mode of practice, intended for occasional short-term replacements, has developed in recent years as vacancies have multiplied, particularly in Ile-de-France. With the same difficulty, in establishments which suffer cruelly from the lack of caregivers: this slightly better paid mode of exercise, with great freedom of planning, now competes with traditional recruitment. And it contributes, by gaining momentum, to the disorganization of services.

Article 7 of the proposed law on access to care carried by the deputy (Horizons) for Seine-et-Marne Frédéric Valletoux, supported by the government, wants to stem this trend by targeting young professionals. The text, which must be examined by the joint committee on Thursday, December 7, plans to prohibit temporary employment at the start of their careers for young doctors, nurses and caregivers.

If the measure is adopted, the duration of the ban will be specified by decree – three to four years were initially envisaged. “Temporary work must once again become a marginal phenomenon, it must not be a career modeldefends Frédéric Valletoux. We must send this signal because today we have young people who start with temporary work and continue their careers like that. »

On this phenomenon, as on temporary medical work, there is no detailed data. According to a note from the direction of research animation, studies and statistics (Dares) dating from September, the recourse hospitals to this temporary instructions for use remains ” weak “ compared to other sectors of the economy, but it is “clearly increasing” : over the last six years, the recourse rate went from 0.2% to 0.4%. This is largely explained by temporary nursing, which has seen significant growth since 2021. Between the start of 2017 and the start of 2023, the share of temporary workers in nursing employment increases from 0.6% to 1.5%. in private hospitals and from 0.1% to 0.7% in public hospitals.

Challenge of “replenishing the workforce”

Ile-de-France is one of the regions most affected, according to health stakeholders. At the Assistance publique-Hôpitaux de Paris (AP-HP), temporary work represents around 1,000 positions (full-time equivalents), all qualifications combined and mainly in the paramedical ranks, according to the management of the group of thirty-eight hospitals; and a budget of 90 million euros in 2022, or 20 million more than two years ago.
