Negotiations for the “medical agreement”, this contract which governs, for five years, the relations between the 113,000 independent doctors and the Health Insurance, are about to start: the “framework letter” by which the Ministry of health kicks off the sequence was communicated, Thursday evening, October 27, to the director general of Medicare, Thomas Fatôme. “This negotiation is essential for our health system, access to care for the French and the future of liberal medicine”, can we read under the signature of the Minister of Health, François Braun, and the Minister Delegate, Agnès Firmin Le Bodo. Four priorities are broken down over five pages; all echo the debates of the moment, in particular on medical deserts, the 6 million French people without a doctor, the coverage of unscheduled care, etc.
It is first a question, therefore, of the fight against inequalities in access to health in all their forms – territorial, social or financial. Secondly, the integration of prevention in the activity of doctors and the improvement of practices and the quality of care. Then comes the release of the “medical time” in order to allow doctors to increase their patient base. Finally comes the challenge of deploying the “digital health”to be put at the service of patients and professionals.
To engage city medicine in the effort requested for 2023-2028, the two ministers promise a “revaluation of the conditions of exercise”. They defend devices “promoting installation and maintenance” in medical deserts, even if it means “optimize aid” existing in return for“strong professional commitments”. They invoke the ” task sharing ” and the “cooperation with other health professions”, but also the “presence of doctors, in particular specialists, outside their usual place of practice”, in conjunction with specialized care teams.

An invitation to aggiornamento? This is not formalized as an obligation – it would offend the profession. “Above all, we would like the solutions implemented (…) come from the actors of the health system themselves, in a logic of proximity, trust and responsibility”, write the ministers. A method also defended within the framework of the National Council of the refoundation-health, inaugurated at the beginning of October. The negotiations that are about to begin will have to integrate the contribution.
