The administration has reduced the federal marketplace rate from 2.5% in 2026 to 1.9% for 2027 to lower ongoing costs. However, health policy experts dispute the framing of the refund pool. Research from the nonpartisan Kaiser Family Foundation indicates that an estimated $1 billion in unspent fees had accumulated before President Joe Biden took office in early 2021, stemming partly from user fees collected during Trump’s first term when marketing outlays were scaled back.

Middle-Class Policyholders Without Subsidies Qualify for Rebates

The rebate targets a specific demographic among the nation’s roughly 19.2 million ACA marketplace enrollees. To qualify, recipients must reside in one of the thirty eligible states, hold a plan through HealthCare.gov, and have paid the full cost of coverage without receiving federal premium tax credits or financial assistance.

Most recipients are middle-class policyholders earning above income thresholds for subsidies—specifically more than $62,600 as an individual or $128,600 for a family of four—though officials noted that some individuals between 100% and 400% of the poverty line who did not receive subsidies will also qualify. Households with multiple eligible members can receive separate payments for each affected person.

Geographic limits leave out residents in twenty states that operate independent exchanges. Policyholders in states such as Colorado, Nevada, and others utilizing state-based platforms do not receive the checks because their state exchanges collect and manage their own fee structures.

Critics Link Rebate Timing to Midterm Elections

Coming just five weeks before the November midterm elections, the timing has drawn pointed criticism from policy analysts and political opponents. The payouts arrive as high costs remain a major liability for Republicans defending congressional majorities.

Critics point out that the checks do not address the financial strain faced by lower-income enrollees who lost enhanced pandemic-era subsidies at the end of 2025, a lapse that caused premiums to rise for millions.

The federal poverty level stands at $15,960 a year for an individual in the contiguous 48 states and Washington, D.C., but the refund program reaches certain individuals who earn between 100% and 400% of that federal poverty level yet received no subsidies. Nevada operates its own ACA marketplace, Nevada Health Link, which generally makes local residents ineligible for the $500 payments. Nationwide enrollment in ACA marketplace coverage sits above 19 million people, implying that the overwhelming majority of participants will not be eligible for the rebate.

The Trump administration stated that it would start sending the $500 checks to 1 million Americans starting Wednesday. Cox observed that Trump’s initiative appears to bypass individuals who dropped coverage because insurance became financially out of reach. Federal authorities disclosed that upwards of 950,000 individuals across 30 states will begin getting $500 disbursements from the Treasury Department as early as this week.
