A resident of the UK fell out of the ambulance when she was taken to the hospital and died.

In the morning, 66-year-old Anita Woodford was taken to the hospital from her home for a dialysis treatment. The patient was placed in the wrong wheelchair, and as she was being unloaded from the ambulance on the hospital grounds, she rolled down the ramp. As a result of the incident, the woman received a head and neck injury and died two weeks later.
At the hearing in her death, the jury was shown video footage of the moment when nursing assistant Tracey Lee rolled Woodford’s wheelchair out of the car. The footage shows the wheelchair rolling down the ramp before tipping over.
Lee told the court that she did not know why it happened the way it did. She stated that she did everything the same as on any other day. The assistant stressed that she never thought that this could happen. However, after an internal investigation, she was fired.
