This treatment involves removing some of the immune cells (T cells) from the patient before injecting the CAR-T cells and transferring the chimeric antigen receptor (CAR) into a special clean room where these CAR-T cells are administered to patients, which binds autoreactive B cells in their blood and destroys them. This causes a reactivation of the immune system.

Since 2021, a group has successfully treated patients with various autoimmune diseases (including SLE) using CAR-T cells. In February 2024, the team presented 15 cases as part of a pilot study in the New England Journal of Medicine; The CASTLE study has 24 participants and is currently ongoing. All those who received the treatment did well, either in good health or with significantly reduced symptoms.

The trick is to find a balance between making sure the chemotherapy works well and making sure kidney function isn’t harmed.
