Now, with Trump back in office, he is determined to fulfill his promise of aligning American drug prices with those in europe and Canada through a policy known as “most favored nation” status.

Why Are US Drug Prices So High?
Despite these challenges, President trump aims to secure more favorable pricing for the United States, comparable to that enjoyed by European nations. On average, Americans pay significantly more for pharmaceuticals than their international counterparts.
The disparity stems, in part, from the negotiating power of other countries. “Other countries negotiate much more effectively than we do, and this is partly as they’re negotiating for the whole country at one time,” Dusetzina noted. Many nations have implemented some form of national health insurance, allowing for unified negotiations with drug companies.
Furthermore, these countries often conduct value assessments, comparing the clinical and economic benefits of new treatments to existing standards of care. They then determine a price based on this value, and are willing to forgo access if companies refuse to comply. “If companies are not willing to agree to that price, these countries will often say, ‘Then, you’re just not going to get to sell yoru drug to anybody in our country.'”
In contrast, the US system is fragmented. “We don’t have a single-payer,” Dusetzina pointed out. “We have Medicare, Medicaid, we have private health insurance plans. It’s not just one group negotiating.” additionally, there’s a strong desire for universal access to all available drugs, even if the cost-effectiveness is questionable.
The Politics and Practicalities of Drug Pricing Reform
The issue of drug pricing is politically popular across the spectrum, yet implementing effective solutions proves difficult. While there’s broad agreement on the need for change, differing views exist regarding who should be responsible.
Republicans generally favor a private market approach, with concerns that heavy-handed price controls could stifle innovation in drug development. dusetzina also highlighted the challenge of translating political promises into tangible policy changes. “Policies are just not as sexy as politics,” she said. “You can have a big, splashy proclamation…that doesn’t really do much.”
Indeed, the initial agreement with Pfizer may not deliver substantial savings for most Americans. According to experts, the average consumer is unlikely to see any immediate change, even those taking Pfizer’s medications. While the prospect of purchasing drugs directly through a website like TrumpRx may appeal to those familiar with saving money on generic drugs, the high cost of branded drugs often makes this option impractical.
