Italy Faces Escalating Vector-borne Disease Threat: West Nile Virus Cases Surge to 740

Italy is confronting a growing public health challenge as cases of West Nile Virus (WNV),Chikungunya,and Dengue fever continue to rise across multiple regions. The Italian National Institute of Health reports a important increase in WNV infections, coupled with ongoing outbreaks of Chikungunya and isolated cases of Dengue, raising concerns about the potential for wider transmission.
West Nile Virus: A Deadly Rise in Infections
Confirmed cases of WNV infection in Italy have reached 740 as of the latest update, marking an increase of 22 cases in the past week. Tragically, the outbreak has resulted in 68 deaths. Health officials are particularly concerned by the high proportion of neuroinvasive cases, where the virus affects the brain or spinal cord.
Of the confirmed WNV cases, 354 have been classified as neuroinvasive, with significant concentrations in several regions: Piedmont (16 cases), Lombardy (53 cases), Veneto (33 cases), Lazio (84 cases), and Campania (81 cases). Additionally, 57 asymptomatic cases have been identified through blood donor screening, 318 cases presented with fever – including imported cases from Kenya, Egypt, and the Maldives – and 7 were symptomatic.
The case fatality rate for neuroinvasive WNV currently stands at 19.2%, a figure that has fluctuated in recent years, reaching 20% in 2018 and decreasing to 14% in 2024.The virus is now circulating in 75 provinces across 17 regions, indicating widespread transmission.
Chikungunya Outbreaks demand Vigilance
Alongside the escalating WNV situation, Italy is battling multiple outbreaks of Chikungunya. From January 1 to October 7, 2025, a total of 398 confirmed cases have been reported, with 45 linked to travel and 353 originating within Italy. The median age of those infected is 60, and approximately 49% are male. Fortunately, no deaths have been attributed to Chikungunya in this period.
five separate episodes of local transmission have been identified in Emilia-Romagna, Veneto, and tuscany. The largest outbreak, affecting 299 individuals, is centered in a municipality within the province of Modena (Emilia-Romagna), where all confirmed cases have exhibited symptoms. A second, smaller outbreak with 50 symptomatic cases is unfolding in several municipalities in the province of Verona (Veneto), though no new cases have been reported in over two weeks. A third,smaller outbreak in Bologna (Emilia-Romagna),involving just two cases,is considered resolved.
Dengue Fever: limited Local Transmission
While less widespread than WNV and Chikungunya, dengue fever is also present in Italy. Between January 1 and September 23, 2025, officials have confirmed 166 cases, with the vast majority (162) linked to travel and only 4 originating locally. The median age of those infected is 38, and 55% are male.No fatalities have been reported.
Two separate instances of local transmission were identified in Emilia-Romagna and Veneto, but both are now considered concluded.
The convergence of these three vector-borne diseases presents a complex challenge for Italian public health authorities, requiring sustained surveillance, robust vector control measures, and continued public awareness campaigns.
