Accurate Ankle Blood Pressure: New Method Improves Readings & Reduces Misdiagnosis

For more than a billion people worldwide living with hypertension, accurate blood pressure monitoring is a cornerstone of preventative care. But what happens when a standard arm reading isn’t possible? A new method, developed by researchers at the University of Exeter Medical School, promises to dramatically improve blood pressure assessment for individuals with limb loss, stroke-related paralysis, or other conditions that prevent traditional measurements. The innovation centers around a personalized predictive model that refines the accuracy of blood pressure readings taken at the ankle, a technique often used as an alternative, but historically less reliable.

The research, published in BMJ Open and funded by the National Institute for Health and Care Research (NIHR), analyzed data from over 33,000 individuals to create a more precise way to estimate arm blood pressure using ankle readings. Crucially, the team has also launched a freely available online calculator – the ABLE-BP Tool – designed to help both healthcare professionals and patients interpret these ankle-based results. This tool represents a significant step toward equitable access to accurate cardiovascular health information.

High blood pressure is a leading risk factor for heart disease, stroke, and kidney problems, making reliable measurement essential for effective diagnosis and management. While the upper arm is the typical site for measurement, an estimated 10,000 adults in the UK live with upper limb loss, according to the NHS, and approximately 75% of the 1.3 million stroke survivors in the country experience upper limb difficulties that can hinder accurate arm-based readings. These individuals often rely on ankle measurements, which tend to be higher than arm readings, potentially leading to misdiagnosis and inappropriate treatment decisions.

Closing the Accuracy Gap in Blood Pressure Measurement

Professor Chris Clark, who led the research at the University of Exeter, explained the potential impact of this new approach. “Our new method will provide a more accurate blood pressure reading for around two percent more people,” he said. “This doesn’t sound like a big number, but remember, around a third of adults have high blood pressure, and once you secure into your 60s, it’s more than half. The NHS Health Check Programme diagnoses 38,000 new cases annually in England alone, so two percent equates to 750 fewer potential misdiagnoses per year in England, and tens of thousands globally.”

The researchers achieved this improvement through statistical modeling, analyzing data from 33,710 people (with an average age of 58 and 45% female) across multiple countries. Their analysis established a predictive equation for estimating arm blood pressure based on ankle readings and examined the correlation between ankle readings and critical health outcomes, such as heart attack risk. The resulting model accounts for individual variations, providing a more personalized and accurate assessment.

Beyond the Numbers: A Personal Impact

The implications of this research extend beyond statistical improvements. For individuals like Sue Kent, a 62-year-vintage TV presenter from Swansea, Wales, accurate blood pressure monitoring has been a long-standing challenge. Kent, who has an upper limb disability caused by the drug Thalidomide, has arms too compact for a standard blood pressure cuff. “I rarely had my blood pressure taken when I was younger,” Kent recounted, “but when I did, they’d utilize a big cuff around my thigh. Whether it was accurate or not, nobody worried.”

However, a recent ankle reading taken during a cataract operation raised concerns. “It was very high, they did it three times, and every time it was high, and it made me very worried,” she said. The uncertainty prompted Kent to seek a private dye injection to assess her blood flow, fearing a potential stroke, as she also lives with Meniere’s disease, a rare inner ear condition. “As you get older, blood pressure is an important indicator of so many things,” Kent emphasized. “You need the right information about your blood pressure.”

Kent’s experience highlights the anxiety and potential for misdiagnosis faced by individuals who cannot have their blood pressure accurately measured using standard methods. She believes this new approach could be transformative. “Why should I not be able to have my blood pressure taken when it’s available to most people?” she asked. “When you’re disabled, you’re more likely to die younger…this resource tips the scales a bit more in our favor.”

Expert Endorsement and Future Directions

The research has garnered praise from leading health organizations. Juliet Bouverie OBE, CEO of the Stroke Association, noted that someone in the UK suffers a stroke every five minutes, with high blood pressure contributing to roughly half of those cases. “Around two-thirds of stroke survivors will leave hospital with some form of disability, including paralysis in an arm, which can prevent getting accurate blood pressure readings from the affected limb,” Bouverie said. “Receiving an accurate blood pressure reading in the ankle will not only provide benefits in the primary prevention of stroke, but importantly in easing the minds of stroke survivors.”

Professor Kevin Munro, Director of NIHR’s Research for Patient Benefit Programme, hailed the research as “an ingenious solution to an important problem.” He added, “Keeping track of blood pressure is a vital tool to help keep people healthy, and this NIHR-funded research will help to spot high blood pressure and treat it even more widely.”

The study, formally titled “Arm Based on LEg blood pressures (ABLE-BP): Can systolic ankle blood pressure measurements predict systolic arm blood pressure? An individual participant data meta-analysis from the INTERPRESS-IPD Collaboration,” was published in BMJ Open.

This advancement represents a significant step toward more inclusive and accurate cardiovascular care. Researchers are continuing to refine the model and explore its potential applications in diverse populations. The ongoing development and wider adoption of tools like the ABLE-BP calculator promise to empower both clinicians and patients, ensuring that everyone has access to the information they need to manage their blood pressure effectively.
