Dr. Bardia pointed out that while the CLEOPATRA regimen involved a transition to maintenance therapy after six to eight cycles of chemotherapy, the current T-DXd labeling and the underlying clinical trial data suggest continuing treatment until disease progression or unacceptable toxicity. Ongoing research, such as the DEMETHER study, aims to provide further clarity on potential maintenance strategies.

  • Gastrointestinal distress: Nausea is the most common side effect, typically managed with a three-drug antiemetic regimen, including olanzapine for breakthrough symptoms.
  • Cardiac monitoring: As with all anti-HER2 therapies, there is a risk of decreased ejection fraction, requiring regular echocardiograms throughout the course of treatment.
