Pennsylvania health officials reported the state’s first two measles-associated deaths in 35 years, both involving unvaccinated individuals in Lancaster County. According to the Centers for Disease Control and Prevention, 92.7% of Pennsylvania kindergarteners received the MMR vaccine before the 2025–26 school year, falling below the 95% threshold required to maintain herd immunity.
State health authorities reported a statewide total of 460 infections, with Lancaster County bearing the heaviest burden at 197 cases. But as the case count climbed, the confirmation of the deaths ignited an extraordinary clash between state leaders and the federal health establishment.

Minutes before a news conference on Tuesday, Governor Shapiro engaged in a heated telephone call with U.S. Health and Human Services Secretary Robert F. Kennedy Jr. Standing before television cameras, the Democratic governor accused the federal health chief of sowing confusion about vaccines and harming public health across the nation.

Kennedy fired back sharply on social media, accusing the governor of fearmongering and the weaponization of infectious disease
for political gain. The federal health secretary suggested that the reported fatalities might have been altogether fabricated
by one of Shapiro’s hopeful staffers.
Kennedy further asserted that the local coroner had no record of any measles-related deaths in the county.
The federal-state feud deepened when the Centers for Disease Control and Prevention claimed that the Governor’s Office had failed to provide relevant outbreak information and had declined federal assistance. The Pennsylvania Department of Health countered immediately, releasing a statement that it was inaccurate to suggest that Pennsylvania has not provided the CDC with information or turned down help, maintaining that state officials regularly share data with federal program staff.
Coroner Review and the Challenge of Counting Measles Deaths
At the center of the dispute is a fundamental question of medical classification: what constitutes a measles death? State health officials rely on epidemiological evidence and positive laboratory testing to designate a fatality as measles-associated,

The infant was too young to receive the standard measles vaccination. While postmortem testing revealed measles antibodies in the child’s lung tissue, the forensic pathologist attributed the immediate cause of death to a lacerated spleen and concluded that the virus did not play a role.
State health officials defended their reporting process. Pennsylvania Health Secretary Dr. Debra Bogen, a pediatrician with more than three decades of experience, emphasized that she thoroughly reviewed the case files and sadly can confirm that there were two recent measles-associated deaths in Lancaster County.
Public Health Warnings and International Stakes
Independent medical experts caution that the political battle has obscured an escalating public health crisis. Dr. Amesh Adalja, an infectious disease specialist and senior scholar at the Johns Hopkins Center for Health Security, warned that it’s no longer about getting answers and figuring this out once a public health issue becomes entirely politicized.
The U.S. eliminated the domestic spread of measles in 2000, but declining vaccination rates have left communities vulnerable to the worst national surge in more than three decades. Physicians note that unimmunized individuals not only risk their own health but also endanger vulnerable populations, including pregnant women, immunocompromised patients, and infants too young for inoculation.
