Surrey and Borders Partnership NHS Foundation Trust has become the first in Europe to join an international clinical trial testing whether the experimental drug trontinemab can prevent Alzheimer’s disease in healthy adults aged 55 to 80 who show no symptoms of memory loss.

Targeting Alzheimer’s Before Symptoms Emerge

Medical researchers are fundamentally shifting their strategy against dementia by moving upstream from treatment to prevention. A new international trial aims to determine if an investigational medicine can stop the destructive condition in its tracks before cognitive decline ever manifests in patients. Surrey and Borders Partnership NHS Foundation Trust initiated screening for the first European participants on Monday, serving as the third site globally to join the study as detailed by the BBC.

The trial is recruiting approximately 1,600 healthy adults worldwide between the ages of 55 and 80. British participants recruited through the Surrey and north east Hampshire trust can originate from anywhere across the United Kingdom.

Screening for Biomarkers and Beta-Amyloid

Before acceptance into the study, candidates undergo rigorous evaluations to confirm they have no existing memory problems. Previous research indicates that individuals displaying the highest levels of p-tau217 face an estimated 78% chance of developing cognitive impairment within a decade, while blood tests measuring the biomarker can identify the disease with up to 95% accuracy.

The study drug itself, named trontinemab, was developed by the Swiss pharmaceutical company Roche. Delivered directly into the vein via an IV bag, the investigational treatment attacks beta-amyloid, a sticky protein that clumps into toxic plaques between brain cells. This accumulation progressively slows down and ultimately destroys neural function. Early-stage research found that trontinemab cleared amyloid plaques in nine out of ten patients with early Alzheimer’s or mild cognitive impairment within 28 weeks.

Trial Structure, Timeline, and Cautious Expectations

The clinical evaluation is expected to run for four to six years. Participants will be randomly assigned to receive either trontinemab or a placebo, then monitored over time to determine how rapidly, if at all, they develop signs of dementia. At least 15 hospitals and clinics across the UK are slated to join the study by the end of the year.

Despite the scientific optimism surrounding plaque clearance, investigators emphasize that the outcome remains entirely unproven. The trial deals with an investigational intervention rather than an assured remedy.

Broader Context in Dementia Research

Alzheimer’s disease accounts for 60% to 80% of the UK’s dementia cases, representing an escalating public health challenge. National estimates indicate that approximately 982,000 people are currently living with dementia across the country. The condition affects roughly one in 14 individuals over the age of 65, a proportion that climbs sharply to one in six for those over 80.

Concurrently, the broader scientific community continues to scrutinize the tools used to measure cognitive performance in clinical settings. Researchers have highlighted persistent limitations in traditional psychometric instruments like the ADAS-Cog test—first published in 1984—suggesting that older evaluation frameworks may struggle to capture the subtle cognitive shifts associated with modern preventative drugs targeting the earliest stages of the disease.
