The VA’s rapid adoption of the Age-Friendly Health System movement is driven by the 4Ms Framework – What Matters, Medication, Mentation, and Mobility. This holistic approach prioritizes the unique needs and preferences of older Veterans, delivering benefits to both healthcare systems and the providers who serve them.

The 4Ms Framework provides a structured approach to addressing the complex health needs of aging Veterans. Veteran’s Journey Home: A Success Story
The transformative power of the 4Ms Framework is vividly illustrated by the story of Manuel Velasco, an 87-year-old Veteran. Mr.Velasco, who uses a wheelchair due to chronic hemiplegia, had maintained his independence at home with his wife until a concerning presentation during a routine follow-up led to an emergency room visit and subsequent placement in a Community Living Center (CLC).
At the CLC, the care team quickly discovered Mr. Velasco’s primary desire: to return home. Despite being completely dependent with his Activities of Daily Living (ADL), he yearned to regain his independence.He expressed feelings of depression and even suicidal ideation, fueled by a fear of permanent institutionalization.
Restoring Independence Through Collaborative Care
Through dedicated effort and the systematic submission of the 4Ms Framework, Mr. Velasco’s wish was granted. The healthcare team secured appropriate equipment, coordinated comprehensive home-based care, and provided crucial support to his primary caregivers. This collaborative approach enabled mr. Velasco to happily return to his home and resume a fulfilling life.
