The relentless pressure to document patient encounters has long been a source of frustration for physicians, contributing to burnout and potentially impacting time spent directly with patients. In recent years, many healthcare systems have turned to artificial intelligence (AI) scribes – programs that listen to doctor-patient conversations and automatically generate summaries – as a potential solution. However, a new study suggests the impact of these tools may be more modest than initially hoped. While AI scribes do offer some time savings, physicians still bear the responsibility of reviewing and approving the generated documentation, limiting the overall efficiency gains.
Published Wednesday in JAMA, the research, led by investigators at Mass General Brigham (MGB) and the University of California, San Francisco (UCSF), found that AI scribes reduced the amount of time clinicians spent on documentation by an average of 16 minutes per day. This seemingly small reduction, however, represents a potential opportunity to see one additional patient every two weeks, according to MGB spokesperson Ryan Jaslow. The benefits were most pronounced for primary care providers, who often face a heavier documentation burden due to the breadth of topics covered in their appointments.
Modest Time Savings, Significant Variation
The study analyzed data from 8,581 clinicians across five healthcare systems – Mass General Brigham, Yale New Haven Health System, Emory Healthcare, University of California at Davis Health, and UCSF Health – comparing those who used AI scribes (1,809 clinicians) with those who did not (6,772 clinicians). Researchers discovered that primary care providers experienced the greatest benefit, saving an average of 27 minutes daily. Even with the assistance of AI, however, these clinicians still dedicated more than two and a half hours each day to documenting patient visits.
The degree of benefit appeared to correlate with usage. Clinicians who utilized AI scribes for more than half of their patient encounters saw the largest reduction in documentation time. However, the study also revealed that only about one-third of doctors who adopted the technology used it consistently to that extent. This suggests that maximizing the potential of AI scribes requires not only implementation but also sustained and comprehensive integration into clinical workflows.
AI Scribes and Physician Burnout: A Complex Relationship
The potential for AI scribes to alleviate physician burnout has been a key driver of their adoption. A previous study, published last August in JAMA Network Open and also co-led by researchers from MGB and Emory University, demonstrated a 21 percent decline in self-reported burnout among physicians using AI scribes. However, Dr. Rebecca Mishuris, chief health information officer at MGB and senior author of the recent JAMA study, cautions against assuming a direct causal link. “The modest reductions in documentation time we observed are unlikely to fully account for changes in burnout,” she stated. Burnout is a multifaceted issue, and while reducing administrative burden is a positive step, it’s unlikely to be a complete solution.
MGB’s Implementation and Expansion of AI Scribe Technology
Mass General Brigham began piloting AI scribe technology in July 2023 with a small group of 18 clinicians to ensure accuracy and reliability in capturing patient visit details. Following a successful pilot phase, the technology was rolled out system-wide last April. As of now, approximately 4,500 clinicians – representing a variety of specialties – out of a total of 7,780 at MGB are actively using AI scribes. The organization has approximately 1,000 primary care providers utilizing the technology.
Dr. Lisa Rotenstein, an associate professor of medicine at UCSF and lead author of the JAMA study, emphasized that certain groups may experience greater benefits. “Certain groups like primary care physicians and physicians who use the scribes more may gain a particular benefit,” she explained. This highlights the importance of tailoring implementation strategies to the specific needs and workflows of different specialties and individual clinicians.
Looking Ahead: Optimizing AI Integration in Healthcare
The findings from this study underscore the potential of AI scribes to streamline documentation processes, but also highlight the necessitate for realistic expectations and strategic implementation. The technology isn’t a panacea, and maximizing its benefits requires consistent use and careful integration into existing clinical workflows. Future research will likely focus on identifying best practices for AI scribe implementation, exploring ways to further reduce the burden of review and approval, and investigating the long-term impact on physician well-being and patient care. MGB continues to monitor the impact of AI scribes and gather feedback from clinicians to refine its implementation strategy.
