A new clinical trial is offering a glimmer of hope in the search for more effective treatments for depression. Researchers have found that dimethyltryptamine (DMT), a psychedelic compound found in the Amazonian brew ayahuasca, significantly reduced depressive symptoms in participants when administered alongside psychotherapy. The findings, published this month in Nature Medicine, add to a growing body of research exploring the potential of psychedelic-assisted therapies.

The phase IIa randomized, placebo-controlled trial involved 34 individuals experiencing moderate to severe depression. Those who received a 21.5-mg dose of DMT, delivered via injection, experienced a greater reduction in depression severity compared to those who received a placebo. Importantly, the effects of the DMT treatment lasted for up to six months for some participants, suggesting a potentially longer-lasting benefit than some existing therapies. This research builds on increasing interest in DMT therapy as a novel approach to mental health.

The study, led by researchers at Imperial College London and sponsored by Cybin UK (now trading as Helus), marks a significant step forward in understanding the therapeutic potential of DMT. Dr. David Erritzoe, Clin Associate Professor in Psychopharmacology & Psychiatry at Imperial College London, cautioned that these are early-stage findings, but emphasized their promise. “Although such early trial results should always be interpreted with some caution, they hold great promise for DMT therapy as a potential treatment for clinical depression,” he said.

A Shorter, More Controlled Psychedelic Experience

Unlike traditional ayahuasca ceremonies, which involve consuming a tea containing DMT and other compounds over several hours, this trial utilized a synthetic DMT formulation administered via injection. This resulted in a much shorter, but intense, 30-minute psychedelic experience. This difference is significant, according to researchers, potentially offering similar therapeutic benefits at a reduced cost and with a more manageable patient experience. The traditional ayahuasca brew often induces nausea and vomiting due to the enzymes that slow the processing of the psychedelic components, a side effect avoided with the synthetic formulation.

However, Dr. Daniel Perkins, a senior research fellow at the University of Melbourne’s psychedelics research and therapeutics unit, notes that the vomiting sometimes experienced during traditional ayahuasca ceremonies can have psychological benefits for some individuals. “In our research, people do report that vomiting can have quite psychologically, emotionally cathartic effects,” he explained, adding that it sometimes occurs at the peak of processing and releasing trauma. However, his team’s research hasn’t shown a significant difference in overall benefit between those who vomit and those who don’t during ayahuasca experiences.

The Importance of Therapeutic Support

Regardless of the method of administration, a key component of successful psychedelic-assisted therapy is the presence of a trained facilitator or therapist. Tommaso Barba, a PhD candidate at Imperial College London and one of the study’s authors, stressed the importance of therapists in preparing patients for the experience, helping them understand it, and integrating it into their lives afterward. “It can act as a catalyst,” Barba said, “because it can help people understand changes they need to produce in their mentality or in their life.”

This therapeutic support is crucial because DMT, like other psychedelics, isn’t a “quick fix.” The improvement in depressive symptoms often comes not just from the drug itself, but from the perform done with a therapist and the patient’s willingness to make difficult life changes. For example, Barba explained, if DMT helps a patient realize their job is contributing to their depression, they would need to actually depart that job to witness lasting improvement.

A Growing Field, Navigating Regulatory Hurdles

The exploration of psychedelics for mental health treatment is gaining momentum. In 2019, the U.S. Food and Drug Administration (FDA) approved Spravato, a ketamine-based nasal spray, for treatment-resistant depression – the first psychedelic-adjacent treatment to receive FDA approval. Trials are also underway for psilocybin (the active component in “magic mushrooms”) and MDMA. However, the FDA declined to approve MDMA-assisted therapy for PTSD due to concerns about data reliability and ethical considerations.

Perkins suggests that psilocybin and DMT may be more appropriate than MDMA for therapeutic use, citing evidence that they can have benefits even outside of a clinical setting. He also noted that MDMA can induce a craving for touch, potentially creating uncomfortable situations with therapists – a risk not associated with DMT or psilocybin.

One challenge facing the field is the regulatory landscape. The FDA regulates drugs, not therapies, which may incentivize pharmaceutical companies to minimize the therapeutic components of these treatments to streamline the approval process. The current DMT therapy requires clinic administration via injection and ongoing therapist support, which may be less appealing to patients than a simple pill.

The researchers emphasize that further investigation is needed to confirm these findings in larger and more diverse populations. The next steps will likely involve larger phase III clinical trials to assess the efficacy and safety of DMT-assisted therapy on a broader scale. The team will also continue to explore the optimal dosage, administration methods, and therapeutic protocols to maximize benefits and minimize risks. The FDA will continue to evaluate data as it becomes available, and the future of psychedelic-assisted therapies will depend on the results of these ongoing studies.
