Low-Dose Radiation Therapy Shows Promise for Knee Osteoarthritis Pain Relief

A new randomized, placebo-controlled clinical trial suggests a potential breakthrough in the treatment of knee osteoarthritis, offering a safe and effective alternative to pain medication and surgery for many sufferers.The study, presented at the American Society for Radiation Oncology (ASTRO) Annual Meeting, demonstrates meaningful pain reduction and improved mobility within four months of a single, low-dose radiation therapy regimen.

Osteoarthritis, affecting an estimated 32.5 million adults in the United States, is a debilitating condition characterized by the breakdown of cartilage in joints. Current treatment options range from lifestyle adjustments and pain relievers to invasive joint replacement surgery.This new research offers a potential middle ground.

Filling a Critical Gap in Osteoarthritis Treatment

“People with painful knee osteoarthritis often face a tough choice between the risks of side effects from pain relievers and the prospect of undergoing major surgery,” explained Dr. Kim, lead author of the study and a radiation oncologist at the University of Pittsburgh Medical Center. “This study suggests that low-dose radiation therapy could provide a meaningful benefit for many patients, potentially delaying or even avoiding these more aggressive interventions.”

The trial involved 100 participants with mild to moderate knee osteoarthritis, randomly assigned to one of three groups: a 3 Gy (gray) radiation dose, a 0.3 Gy radiation dose, or a placebo. Participants were unaware of which treatment they were receiving,serving as the control group. Crucially, the researchers employed a placebo-controlled design, a rigorous methodology often lacking in osteoarthritis trials, to accurately assess the treatment’s efficacy.

Minimal Radiation, Significant Results

A key concern with radiation therapy is potential side effects.Though, this study utilized doses far below those used in cancer treatment. According to researchers, the radiation dose was less than 5% of what is typically used for cancer therapy, and no radiation-related side effects were observed in any participant. This low-dose approach targets the affected joints while minimizing exposure to vital organs.

After four months, the results were compelling.A remarkable 70% of patients in the 3 Gy group met the criteria for a “responder” – defined as achieving meaningful enhancement in at least two of three measures: pain, physical function, and overall condition assessment – compared to 42% in the placebo group (p=0.014). The 0.3 Gy group showed improvement rates similar to the control group (58.3% improved,p=0.157), indicating the 3 Gy regimen was the primary driver of relief beyond the placebo effect.

Moreover, 56.8% of patients in the 3 Gy group reported meaningful improvements in a composite score measuring pain, stiffness, and physical function, compared to 30.6% in the placebo group (p=0.024).

Addressing the Power of placebo and Future Directions

The researchers took steps to isolate the effects of radiation by limiting the use of stronger pain relievers during the four-month follow-up period, allowing onyl acetaminophen as needed. This approach, they argue, strengthens the evidence supporting the efficacy of low-dose radiation.

Interestingly, the study also highlighted the substantial placebo effect observed in osteoarthritis trials. Approximately 40% of patients in the placebo group experienced a positive response, a finding consistent with previous research on injections and medications. “It was surprising, and it underscores how critically important placebo-controlled designs are in osteoarthritis research,” Dr. Kim noted. “We need to examine this more closely in future studies.”

The research team is currently completing a 12-month follow-up to assess the long-term durability of the benefits and to correlate symptom relief with imaging-based measures of joint structure. Future studies will also focus on larger, pragmatic trials to evaluate outcomes in specific patient subgroups and conduct health-economic analyses comparing low-dose radiation to existing treatments like injections and medication regimens.

A new Option for Managing Knee Pain

Dr. Kim emphasized that low-dose radiation therapy should be considered as part of a complete treatment plan,alongside established approaches like weight loss,physiotherapy,and medication. “In clinical practice, responses could be even stronger when radiation is properly combined with othre treatments, and patient satisfaction might potentially be higher than with current options alone.”
