But this success brings a new set of challenges, as researchers start to understand the long-term effects of treated HIV on the body – particularly, its potential impact on brain health and the risk of neurodegenerative diseases. The emerging field of HIV and neurodegeneration is focused on understanding how lifelong viral control affects cognitive aging and whether it increases susceptibility to conditions like Alzheimer’s disease.

Dr. Andrea Gramatica, an immunologist and VP of Research at amfAR, explains that the focus has shifted. “We’re no longer talking about the acute neurocognitive problems that were so common in the early days of the epidemic,” she said in a recent discussion with Dr. Kelsey Hopland, a neuroscientist and new Program Officer, Research, at amfAR. “Now, we’re asking: what does it mean for someone to live with HIV for 40 or 50 years? How does that impact their cognitive trajectory as they age?”

One key area of investigation is the role of chronic inflammation. Even with ART suppressing viral replication, some level of immune activation often persists. This chronic inflammation is increasingly recognized as a driver of many age-related diseases, including neurodegenerative conditions. Researchers are exploring whether the persistent inflammation associated with HIV could accelerate cognitive decline or increase the risk of Alzheimer’s disease.

Unpacking the Link Between HIV, Inflammation, and Alzheimer’s

The connection between HIV and Alzheimer’s disease is complex and not yet fully understood. Alzheimer’s is characterized by the accumulation of amyloid plaques and tau tangles in the brain, leading to neuronal damage and cognitive impairment. While HIV doesn’t directly cause these hallmarks of Alzheimer’s, research suggests it may create an environment that makes the brain more vulnerable to them.

Dr. Kelsey Hopland highlights the potential for shared pathways. “There’s a lot of overlap in the biological processes that are disrupted in both HIV and Alzheimer’s,” she notes. “We see inflammation, oxidative stress, and changes in brain metabolism in both conditions. It’s possible that HIV could exacerbate these processes, accelerating the development or progression of Alzheimer’s.”

Studies have shown mixed results regarding the prevalence of Alzheimer’s disease in people living with HIV. Some studies have suggested an increased risk, while others have found no significant difference. This inconsistency may be due to variations in study design, the populations studied, and the duration of HIV infection and ART exposure. Further research is needed to clarify the relationship.

Current Research and Future Directions

Researchers are employing a variety of approaches to investigate the intersection of HIV and brain health. These include:

  • Longitudinal studies: Following individuals with HIV over time to track cognitive function and identify risk factors for decline.
  • Brain imaging: Using techniques like MRI and PET scans to assess brain structure and function.
  • Biomarker research: Identifying biological markers that can predict cognitive decline or Alzheimer’s disease risk.
  • Animal models: Studying the effects of HIV on the brain in animal models to gain insights into disease mechanisms.

One promising area of research involves the utilize of neuroimaging to detect subtle changes in brain structure and function that may precede clinical symptoms of cognitive impairment. This could allow for earlier intervention and potentially slow the progression of neurodegenerative diseases.

Another focus is on developing strategies to reduce chronic inflammation in people living with HIV. This could involve optimizing ART regimens, exploring the use of anti-inflammatory medications, or promoting lifestyle interventions such as diet and exercise.

The field is also recognizing the importance of addressing comorbidities – other health conditions that often occur alongside HIV, such as cardiovascular disease and diabetes – which can also contribute to cognitive decline.

As Dr. Gramatica emphasizes, this is a rapidly evolving area of research. “We’re still in the early stages of understanding the long-term effects of HIV on the brain,” she says. “But the more we learn, the better equipped we’ll be to develop strategies to protect brain health and improve the quality of life for people living with HIV.”
