Global measles cases surged to 10.3 million in 2023, a 20% increase from the previous year, according to new estimates released by the World Health Organization (WHO) and the U.S. Centers for Disease Control and Prevention (CDC). This alarming rise underscores a growing threat to global health, driven by declining immunization rates and leaving millions of children vulnerable to a highly contagious and potentially deadly disease. The resurgence isn’t uniform; while some regions are experiencing particularly severe outbreaks, the trend signals a worrying global backslide in measles prevention efforts.
Measles, despite being preventable with two doses of the measles vaccine, continues to claim lives and cause serious complications, particularly in young children. In 2023, an estimated 107,500 people died from measles, the majority of whom were under the age of five. While this represents an 8% decrease in deaths compared to 2022, experts caution that the reduction is largely attributable to the geographic distribution of cases, with outbreaks occurring in areas where children have better access to healthcare and nutrition, rather than a fundamental improvement in prevention. The virus remains a significant public health concern, capable of causing pneumonia, encephalitis, and even blindness.
The primary driver of this increase is inadequate vaccine coverage. Globally, 83% of children received their first dose of the measles vaccine in 2023, but only 74% received the crucial second dose. Achieving and maintaining at least 95% coverage with two doses is essential to establish herd immunity and protect communities from outbreaks. “The number of measles infections are rising around the globe, endangering lives and health,” stated CDC Director Mandy Cohen. “The measles vaccine is our best protection against the virus, and we must continue to invest in efforts to increase access.” The factors contributing to this decline in coverage are complex and vary by region, including vaccine hesitancy, disruptions to healthcare systems caused by conflicts and the COVID-19 pandemic, and logistical challenges in reaching remote or underserved populations.
The Threat to Elimination Goals
The current trajectory of measles cases poses a serious threat to global elimination goals. As of the end of 2023, 82 countries had achieved or maintained measles elimination, meaning the virus was no longer endemic within their borders. Recently, Brazil was re-verified as having eliminated measles, a positive development for the WHO Americas Region. However, with the exception of the African Region, the progress is fragile and susceptible to reversal. Maintaining elimination requires sustained high vaccination coverage and robust disease surveillance systems.
The Immunization Agenda 2030, a global strategy to strengthen immunization programs, aims to prevent outbreaks and protect populations from vaccine-preventable diseases like measles. However, achieving these goals requires urgent and targeted interventions, particularly in regions with the lowest vaccination rates and the most vulnerable populations. These interventions include strengthening routine immunization programs, conducting targeted vaccination campaigns, and improving disease surveillance to quickly detect and respond to outbreaks.
Addressing the Root Causes and Strengthening Surveillance
Effective measles control requires a multi-faceted approach. Countries and global partners must prioritize increasing vaccine access, addressing vaccine hesitancy through education and community engagement, and strengthening healthcare systems to ensure that all children have access to immunization services. The Global Measles Rubella Laboratory Network (GMRLN) plays a crucial role in this effort by providing laboratory support for measles surveillance and outbreak investigation. Strong surveillance systems are essential for identifying outbreaks early, tracking the spread of the virus, and evaluating the effectiveness of vaccination programs.
