A new immunotherapy drug, VIR-5500, is showing remarkable promise in early clinical trials for advanced prostate cancer, offering a potential breakthrough in treating a disease that affects millions worldwide. The drug, designed to activate the body’s own immune system to fight cancer, has demonstrated significant tumor shrinkage in a substantial portion of patients who had stopped responding to conventional treatments. This development offers renewed hope for men facing advanced stages of the disease, where treatment options are often limited.
Prostate cancer is the most commonly diagnosed cancer among men in many countries, including the United States and the United Kingdom. Approximately 1.5 million men are diagnosed globally each year, according to the American Cancer Society. While immunotherapy has revolutionized cancer treatment in recent years, its effectiveness against prostate cancer has been historically limited. VIR-5500 appears to overcome some of those hurdles, presenting a new avenue for therapeutic intervention.
The phase one clinical trial, led by Professor Johann de Bono of the Institute of Cancer Research, London, and The Royal Marsden NHS Foundation Trust, involved 58 men with advanced prostate cancer. The study, funded by Vir Biotechnology, focused on assessing the safety of the drug and determining the optimal dosage. Results presented at the American Society of Clinical Oncology (ASCO) Genitourinary Cancers Symposium in San Francisco revealed that 88% of patients experienced only mild side effects, a significant improvement over other T-cell engagers which have been known to cause severe inflammatory responses.
A ‘Cloaking Device’ for Targeted Treatment
What sets VIR-5500 apart is its innovative “cloaking device,” explained Professor de Bono. This feature ensures the drug remains inactive until it reaches the tumor site, minimizing off-target effects and reducing the risk of adverse reactions. The drug functions as a T-cell engager, binding both to the body’s immune T-cells and to prostate-specific membrane antigen (PSMA) found on the surface of cancer cells. By bringing these two elements together, VIR-5500 facilitates the targeted destruction of cancer cells.
The researchers analyzed prostate-specific antigen (PSA) levels in the patients’ blood – a key biomarker for prostate cancer. For 17 men receiving the highest dose of VIR-5500, 14 (82%) saw their PSA levels fall by at least half. Nine (53%) experienced a reduction of at least 90%, and five (29%) saw a fall of at least 99%. Of the 11 patients with measurable tumors, five exhibited tumor shrinkage. In one particularly striking case, a 63-year-traditional man whose cancer had spread to his liver experienced the complete resolution of 14 cancerous lesions after six cycles of treatment.
Unprecedented Results for an ‘Immune-Cold’ Disease
Professor de Bono described the results as “unprecedented” for a disease previously considered “immune-cold,” meaning resistant to immunotherapy. “We believe that such treatments may in the long term lead to cures,” he stated. The drug’s ability to linger in the bloodstream for a longer duration, thanks to the cloaking mechanism, also suggests that fewer doses may be required, potentially improving patient convenience and reducing treatment costs.
While the findings are highly encouraging, experts caution that the trial is still in its early stages and the results have not yet been peer-reviewed. Charlotte Bevan, a professor of cancer biology at Imperial College London, who was not involved in the study, emphasized the importance of conducting further research with diverse patient populations. “It is vital studies are carried out with patients of different ethnicities, as there were disparities in prostate cancer outcomes,” she noted.
Looking Ahead: Larger Trials and Broader Access
Simon Grieveson, assistant director of research at Prostate Cancer UK, hailed the early-phase trial as “exciting,” highlighting the urgent need for innovative treatments. “With over 12,000 men dying from prostate cancer each year in the UK, we urgently need new and innovative ways to treat the disease,” he said. “These early results are extremely promising…I look forward to seeing this now tested in larger trials, with the hope that this treatment will offer men more valuable time with their loved ones.”
Further clinical trials are now being planned to confirm these initial findings and assess the long-term efficacy and safety of VIR-5500. The success of these trials will be crucial in determining whether this novel immunotherapy can become a standard treatment option for advanced prostate cancer, offering a significant improvement in outcomes for patients facing this challenging disease. The next steps involve expanding the trial to include a larger and more diverse patient cohort, and rigorously evaluating the drug’s performance across different stages and subtypes of prostate cancer.
